Computer-assistierte retrograde Anbohrung der Osteochondrosis dissecans tali mittels fluoroskopischer Navigation

Abstract: Thanks to the significantly increased accuracy, fluoroscopic navigation offers a high degree of safety and efficacy for this minimally invasive procedure. The operation can easily be performed successfully causing only minimal collateral damage to the bone, preserving the joint surface. The inherent risks of the retrograde drilling of osteochondritic lesions are lower with navigation, while the radiation exposure of the patient and the staff is significantly reduced.

“…Despite emerging and spreading systems and use, only few studies exist [17][18][19][20]22,[25][26][27][28] that report clinically acceptable results in precision with a distance from the given target of below four millimeters [17][18][19]23,24,26,27]. However, the bending of a wire or the tip of a thin drill could falsify the measured distances from the target since just their length and their indirect orientation by positioning of the drilling guide are detected.…”

“…Computer-assisted orthopedic surgery (CAOS) meanwhile is well accepted in several surgical fields, for improving precision and also minimizing X-ray exposition [18][19][20][21]. Different methods like the CT-based, fluoroscopically-assisted or imageless methods are used to simultaneously generate different planes of the therapeutic object to be treated [21].…”

“…Drilling is required mainly for three purposes: to provide access to the location of intervention, to guide and position screws, implants and other medical devices, and to create channels for various therapeutic reasons [1]. These different applications of drills presently use linear drilling, i.e.…”
