Introduction:Hypertrophic and keloid scarring remain notoriously troublesome for patients to tolerate and frustratingly difficult for clinicians to treat. Many different treatment modalities exist, signifying the failure of any method to achieve consistently excellent results. Intralesional cryotherapy is a relatively recent development that uses a double lumen needle, placed through the core of a keloid or hypertrophic scar, to deliver nitrogen vapour, which freezes the scar from its core, outwards.Methods:This article provides a comprehensive review of the literature on intralesional cryotherapy for hypertrophic scars and keloids. A systematic review or meta-analysis was not possible, since the existing articles did not permit this.Results:A search of English language, peer-reviewed literature was carried out. The evidence base was found to be low (level 4). In addition, much of the published evidence comes from a very few groups. Despite this, consistent findings from case series suggest that the technique is safe and achieves good scar reduction with very few treatments. Adverse effects include depigmentation, recurrence and pain. Pain and recurrence appear to be uncommon and depigmentation may be temporary.Discussion:Well-constructed, prospectively recruited comparative trials are absent from the literature. These are strongly encouraged, in order to strengthen general confidence in this technique and in the repeatability of outcomes reported thus far.

Background: Keloids are benign fibroproliferative tumors that extend beyond the original wound. There are many different medical and surgical modalities to treat keloids. Objective: This article provides a summary and review of the medical and the surgical options available in the literature for treating keloids. Furthermore, this paper organized the data into monotherapy versus combined therapy. Methods: A literature review was conducted using PubMed and MIDLINE that included English publications trials and reviews from April 2005-June 2018. Results: Monotherapy is unable to completely flattened keloids and a combination therapy is always needed. Monotherapy shows success with the intralesional steroid injection, but there is always a room for combination with surgical excision, or other therapies: 5-fluorouracil (5-FU), bleomycin and interferon, topical imiquimod, compression, cryotherapy, radiation, silicon sheeting and lasers or light-based therapies. Conclusion: The treatment of keloid is tested by a big number of trials using different regimens proving the resistance nature of keloid.

Hypertrophic scars (HTSs) and keloids are a major health concern for aesthetic and functional reasons. Despite a plethora of rapidly evolving treatment options and technical advances, the management of pathologic scarring remains difficult. The development of standardized treatment algorithms has been problematic for years due to the lack of sound randomized controlled trials. Expert panels are more and more establishing guidelines to provide an evidence-based framework on a national and international level. This article aims to evaluate the current strategies and upcoming trends in the therapy and prevention of unpleasant scars and keloids from a clinical perspective. There is strong evidence to support a growing role of early combination treatments, particularly the application of 5-fluoruracil adjunct to intralesional steroid injections. Furthermore, the use of fractional ablative laser technologies such as the CO2 laser has recently yielded promising results with respect to aesthetic outcomes and patient satisfaction at tolerable side effects.
