Comparison of Treatment Modalities for Epidermal Nevus: A Case Report and Review

Abstract: For the past 2 years, we have been treating a teenage girl for a disfiguring, extensive, inflammatory linear verrucous epidermal nevus (ILVEN). In this one patient, we examined the efficacy of numerous treatment modalities and compared our results with those reported in the literature. In our experience, cryotherapy using liquid nitrogen proved to be the most successful therapy, and we have subsequently used it to treat almost all of this patient's lesions. Although cryotherapy does not provide an easy or dram… Show more

“…Although, several treatment modalities have been reported for the cutaneous lesions, such as surgical excision, laser ablation, cryotherapy, dermabrasion, chemical peels, topical corticosteroids and systemic steroids, no information are available for oral lesions [19,20]. In our Case 2 the surgical removal of the palatal nodule was not followed by recurrence, according to the outcomes of other Authors [6,9].…”

“…ILVEN is relatively difficult to treat. Partial improvement however has been reported with topical and intralesional high-potency corticosteroids [2, 9-11], reti noids [12], podophyllin ointment [13] and cryotherapy [10], Familial occurrences of ILVEN have also been reported [8, 14,15]. The mother and daughter described by Hamm and Happle [8] had a bilateral systemized distribu tion of verrucous lesions similar to our patients, and systemized linear lesions similar to ILVEN have been reported in a girl whose sister had the polydysplastic CHILD syndrome.…”

“…In 1983, Fox and Lapins [3] applied various approaches to treating epidermal nevi on one patient who had extensive, disfiguring epidermal nevus lesions. They concluded that cryosurgery using liquid nitrogen was the most successful therapy.…”

“…Topical modalities, including steroids (topical or intralesional), retinoids, calcipotriol, toxins (e.g. podophyllin), antiproliferatives (5-fluorouracil cream), keratolytics, and chemical peels, have shown inconsistent efficacy and a high rate of recurrence, and destructive methods, including laser, electrocoagulation, and dermabrasion, have not uniformly provided acceptable efficacy and side effects and a high rate of recurrence [3,4,5,6,7,8,9,10,11]. Surgery is an option for small lesions, but in large lesions the scar is not necessarily a worthwhile trade-off.…”
