Comparison of the use of Alvarado and AIR scores as an adjunct to the clinical diagnosis of acute appendicitis in the pediatric population

Abstract: BackgroundAcute appendicitis is one of the most common causes of acute abdominal pain with an incidence of 1.17 per 1000 and lifetime risk of approximately 7%. It remains the most common indication for emergency abdominal surgery in childhood. Diagnosis of acute appendicitis is particularly difficult in young women and the pediatric population. In the USA, CT imaging is used to avert diagnostic dilemma, however the procedure is associated with radiation risk in this vulnerable population. Additionally, the pro… Show more

“…However, with due attention to our results, we failed to report an extreme superiority for the AIR scoring system discriminating power in pediatric patients compared to that of the Alvarado system. Although our findings were consistent with a previous study by Musbahi et al , our results were not consistent with the results of Macco et al [ 33 , 34 ]. Also, they revealed a 14% rate of missed appendicitis, which is not only two-fold higher in comparison to the missed cases evaluated by the Alvarado score but also increases the risk of development of life-threatening complications of appendicitis in children.…”

“…To our knowledge, this is one of the first studies which have prospectively compared the accuracy of the AIR and Alvarado scores in the estimation of appendicitis development in pediatric patients [ 33 , 34 ]. According to the study results, both of the scoring systems evaluated in the current survey provide acceptable diagnostic accuracy in ruling out the probability of appendicitis among young individuals due to their higher sensitivity and specificity followed by an acceptable discriminative value.…”

“…It was pointed out by Anderson that it is recommended to repeat the scoring system during observation of patients with persistent symptoms and biochemical markers to come to a diagnostic decision [ 45 ]. The recent guidelines also suggest that the AIR and the AAS significantly aid in clinical diagnosis and have a potential for reducing negative appendectomy rates as well as limiting the use of imaging [ 10 , 46 , 47 , 48 , 49 ].…”
