Comparison of Methods to Detect and Measure Glaucomatous Visual Field Progression

Abstract: Purpose: To compare methods to assess visual field (VF) progression in glaucoma. Methods: 4,950 VFs of 253 primary open angle-glaucoma patients were evaluated for progression with the following methods: clinical evaluation, guided progression analysis (GPA), mean deviation (MD), and visual field index (VFI) rates, Advanced Glaucoma Intervention Study (AGIS) and Collaborative Initial Glaucoma Treatment Study (CIGTS) scores, pointwise linear regression (PLR), permutation of PLR (PoPLR), and glaucoma rate index (… Show more

“…This is an ongoing challenge even in current clinical practice and will further impact the generalizability of innovations derived through AI to individual clinical settings. 109 111 In addition, considerable ocular variability occurs within patient populations depending upon factors, such as age, gender, refractive error, medical comorbidities, and ethnicity. Before ML strategies can be translated to everyday clinical practice, further validation across diverse global patient populations is necessary.…”

“…A VF series was defined as progressing if there were more than or equal to three deteriorating test locations after accounting for improving locations. Because VF progression greatly depends on the method used to define progression, 20 , 21 we also repeated all the analyses using another pointwise PLR criterion (≥ 4 deteriorating test locations with a rate of change ≤ −1 dB/year at P < 0.05) as well as the regression of the global index MD against time (progression defined as negative rate of change at P < 0.05).…”

“…To better control the specificity for detection of change with structural and functional measures, we applied permutation analyses and estimated rates of change at macular superpixels (GCC thickness) and VF test locations (TD values) 5000 times for followup periods from 2 to 5.5 years. 30 We defined the 95% confidence intervals for the individual estimated slopes and considered rates of change falling below the 2.5% cutoff point as representing "true" worsening.…”
