Anestezist için artroplastide risk belirlemesi, preoperatif hastanın kullandığı ilaçların düzenlenmesi, kullanılacak anestezi ve analjezi yöntemleri çok önemlidir ve cerrahinin başarısına etkisi büyüktür. Bu nedenle; artroplastide güncel bilgilerin ışığında bu konuların derlemesi sunulmuştur.Anahtar Kelimeler: Artroplasti, preoperatif değerlendirme, risk belirlemesi, anestezi, analjezi Risk assesment, preoperative drug regulation, the anesthesia and analgesia techniques are very important and the effectivity on success of surgery is grade. So, these topics in arthroplasty were review under current knowledge. 188A rtroplasti; ekleme ağrısız hareket sağlamak ve eklemi kontrol eden yapılara optimal fonksiyonu kazandırmak için implant yerleştirilerek eklemi oluşturan kemik yapılara yeniden yüzey oluşturma girişimidir. Artroplastide Risk DeğerlendirmesiArtroplastide mortalite ve morbidite riski taşıyan hastaların belirlenmesi, intraoperatif ve postoperatif komplikasyonların tahmin edilmesi ve önlenmeye çalışılması, dikkatli preoperatif hazırlık ve postoperatif bakım sağlanması başarı oranını artı-rarak, sağlık harcamalarını büyük oranda azaltır (1). Hastanın yaşı, kardiyovasküler hastalık ve kronik obstrüktif akciğer hastalığı (KOAH) varlığı, geçirilmiş tromboembolik olaylar, böbrek fonksiyonları ve kullanılan ilaçların sorgulanması ve fonksiyonel kapasitenin belirlenmesi, perioperatif yaşanabilecek komplikasyonların sıklığı hakkında önemli bilgiler verir (2). Fizik muayene hastada önemli durumları açığa çıkarmak için çok önemlidir. Vital bulgular hipo veya hipertansiyon, bradi veya taşikardi, aritmi, hipoksi, juguler ven distansiyonu, S3 gallop ritmi, kalpte üfürümler, karotis, femoral veya abdominalde duyulan anormal oskültasyon bulgularının varlığı araştırılmalıdır. Ameliyat öncesi tam kan sayımı, hemoglobin, elektrolitler, kan glikoz düzeyi, üre, kreatin, karaciğer enzimleri, kanama pıhtılaşma zamanları, idrar tetkikleri yapılmalı, akciğer grafisi ve elektrokardiografi (EKG) görülmelidir. EKG'de iskemi ve sol ventrikül hipertrofisi bulguları veya sistolik üfürüm var ise ekokardiyografi istenmelidir. Ekokardiyografide risk belirlemek için en önemli bulgu ejeksiyon fraksiyonudur, %35'in altında olması yüksek risk göstergesidir. Kalpte komplikasyonlar artroplasti sırası ve sonrasında görülen mortalite ve morbidite nedenlerinin başında gelir. Yüksek American Society of Anaesthesiologists (ASA) değerleri ve fonksiyonel kapasitenin azalmış olması en önemli risk faktörleridir. Son 30 yılda nonkardiyak cerrahide perioperatif kardiyak mortalite ve morbidite insidansını belirleyebilmek için Lee İndeksi, Revize Kardiyak İndeks ve Modifiye Goldman İndeksi gibi birkaç risk indeksi gelişti-rilmiştir.
