Since communication skills contribute significantly to professional success among veterinarians, there is a particular focus on developing communication classes in veterinary curricula. At Freie Universität Berlin, an e-learning course covering the basics of communication and a practical communication course based on role plays with and without simulation persons have been established. The outcome of these communication courses on the assessment of the relevance of several veterinary competencies and on the self-assessment of communication skills using the SE-12 questionnaire was investigated. For this purpose, students were surveyed before and after the e-learning course as well as before and after the practical course. Veterinarians were also surveyed on the relevance of veterinary competencies. The relevance of communicative competencies for professional success was rated significantly higher by the students after completing the practical course than by the other students and the veterinarians. Self-assessment of communication skills showed little increase after the e-learning course, but a significant increase after the practical course. Thus, an effective outcome of the communication classes was observed mainly after the practical course. However, the effect of the e-learning course cannot be ruled out since the students participating in the practical course have also completed the e-learning course beforehand.

Learning communication skills is central in medical education. Clinical simulation provides conditions to foster comprehensive teaching and learning processes, although it is necessary to understand student acceptance as a way to evaluate and improve the strategy. The objective of this study was to analyze the perception of medical students after introducing clinical simulation sessions to optimize the teaching and learning processes of communication skills. The study design is observational, cross-sectional, mixed, and uses census sampling. A survey was conducted through a questionnaire, and a descriptive analysis of the results was performed. The results reveal a generally good perception, where the majority of students highlighted the debriefing space or final reflection (64.8%), while 27.8% valued clinical simulation as a space for applying knowledge and more than 20% referred to the simulation scenario or the experience with a simulated patient. In conclusion, the most notable space was debriefing as an area for reflection and learning, although other appreciated aspects such as the application of knowledge or the use of a simulated patient are also important in understanding clinical simulation as a tool for learning communication skills.

Zusammenfassung: Hintergrund: Die neue psychotherapeutische Approbationsordnung sieht erstmals eine anwendungsorientierte Parcoursprüfung vor, in der die therapeutische Handlungskompetenz im Umgang mit Simulationspersonen in fünf Stationen jeweils à 20 Minuten geprüft wird. Fragestellung: Die Pilotstudie, die in Kooperation mit dem IMPP durchgeführt wurde, diente der explorativen Untersuchung des Aufwands, des Schwierigkeitsgrades sowie der Durchführbarkeit und Reliabilität der neuartigen Prüfungsform innerhalb der psychotherapeutischen Ausbildung. Methode: Es wurden zwei Parcours mit jeweils fünf Stationen online mit 39 Studierenden am Ende des dritten Semesters des Masterstudiengangs „Klinische Psychologie und Psychotherapie“ entsprechend der Vorgaben der neuen Approbationsordnung durchgeführt. In jeder Station wurde die therapeutische Handlungskompetenz durch zwei Prüfende anhand vorgegebener Bewertungsbögen bepunktet. Als Bestehensgrenze wurde eine Prüfungsleistung von mindesten 60 % der maximalen Punktzahl in jeder einzelnen Station festgelegt. Die Beurteilungsübereinstimmung innerhalb der einzelnen Stationen wurde mittels Intraklassen-Korrelation (ICC) ermittelt. Für den Gesamtwert der Prüfungsleistung wurden die interne Konsistenz mittels Cronbach’s Alpha sowie der Generalizability-Koeffizient ( G) bestimmt. Ergebnisse: Die 39 geprüften Studierenden erreichten im Mittel 346.29 Punkte ( SD = ± 46.77) von maximal 500 Punkten. Die Durchfallquote betrug damit 59 %. Die Interrater-Reliabilität in den einzelnen Stationen variierte zwischen .091 und .733. In den meisten Stationen war sie befriedigend. Die interne Konsistenz des Gesamturteils betrug α = .85. Der Generalizability-Koeffizient lag für die gesamte Prüfung bei G = .81. Der Gesamtaufwand der Prüfungsdurchführung innerhalb der Hochschule wurde anhand von benötigten Arbeitsstunden bemessen und lag bei ca. 680 Stunden. Diskussion: Es zeigten sich erste Hinweise auf die Qualität der neuen Prüfungsform. Insbesondere war die vorhandene Varianz in den Bewertungen im Wesentlichen auf die Leistung der Studierenden zurückzuführen. Gemeinsam mit den Werten zur internen Konsistenz und zur Prüfenden-Übereinstimmung kann dies als Anzeichen für eine vorhandene Objektivität / Reliabilität der beiden Parcours – im Rahmen der Pilotierung an diesem Standort – gedeutet werden. Die hohe Durchfallquote erfordert weitergehende Untersuchungen. Des Weiteren wäre eine Reduzierung des hohen Aufwandes der Prüfung in dieser Form wünschenswert. Limitationen der Untersuchung sind insbesondere in der geringen, nicht repräsentativen Stichprobengröße zu sehen.
