Colonic Volvulus in the United States

Abstract: Colonic volvulus is a rare cause of bowel obstruction in the United States and is associated with high mortality rates. CV and SV affect different populations and the incidence of CV is on the rise. The presence of bowel gangrene and coagulopathy strongly predicts mortality, suggesting that prompt diagnosis and management are essential.

“…Volvulus is rare in developed countries and accounts for only 1.90% of all cases of bowel obstruction in the United States, but it is a major cause of bowel obstruction in Africa, the Middle East, and South America 9. SCV usually occurs in older adults, with a mean age of 70 years at presentation 3. Patients are often institutionalized and debilitated due to underlying neurologic or psychiatric disease, and tend to have a history of constipation 7.…”

“…Although sigmoid colon volvulus (SCV) is a relatively rare disease, its epidemiology, symptoms, diagnostic methods, treatments, and outcomes have been well researched 34. SCV can compromise the blood supply of the involved segment, leading to ischemia, gangrene, perforation, and death in the absence of optimal treatments 56.…”

“…In the US, about 60% of LBO are caused by neoplasms, 20% by diverticular disease, and 5% due to colonic volvulus [1], [2], [3]. This usually presents in older adults around the age of 70 [4], [5].…”

“…This was analogous to Halabi and colleague’s coding for colonic volvuli. [13] However, since this diagnosis code is not specific for the volvulus location, we then excluded all patients undergoing gastric or colonic procedures during the same hospitalization in order to identify patients undergoing small bowel procedures during the same hospitalization. By excluding gastric and colonic procedures associated with gastric volvuli and colonic volvuli,[13] we aimed to identify the study cohort with SBV (Supplemental Table 1).…”

“…[13] However, since this diagnosis code is not specific for the volvulus location, we then excluded all patients undergoing gastric or colonic procedures during the same hospitalization in order to identify patients undergoing small bowel procedures during the same hospitalization. By excluding gastric and colonic procedures associated with gastric volvuli and colonic volvuli,[13] we aimed to identify the study cohort with SBV (Supplemental Table 1). Although this approach may underestimate the true SBV population, which may have undergone gastric or colonic procedures, we preferred this potential type II error as opposed to overestimation with a type I error.…”
