Abstract: BackgroundOne-size-fits-all interventions reduce chronic low back pain (CLBP) a small amount. An individualised intervention called cognitive functional therapy (CFT) was superior for CLBP compared with manual therapy and exercise in one randomised controlled trial (RCT). However, systematic reviews show group interventions are as effective as one-to-one interventions for musculoskeletal pain. This RCT investigated whether a physiotherapist-delivered individualised intervention (CFT) was more effective than ph… Show more

“…Causal mediation analysis of a multicentre RCT in Ireland. This trial (O'Keeffe et al, 2019;O'Keeffe, O'Sullivan, Purtill, Bargary, & O'Sullivan, 2015) (NCT02145728) was a twogroup, pragmatic RCT in which 206 people with CLBP were recruited from three sites (one public hospital and two primary care centres) in Ireland with follow-up at post intervention, 6 months post randomization and 12 months post randomization. A total of three physiotherapists (one in each setting) were chosen to deliver both the CFT intervention and the group exercise and education intervention in this trial.…”

“…The hypothesized mediators measured were pain self-efficacy, stress, fear of physical activity, coping, depression, anxiety and sleep measured at 6-month follow-up. Due to the pragmatic nature of the CFT intervention, the treatment period varied, such that the post intervention measures for the CFT and group intervention were conducted at different time points (O'Keeffe et al, 2019). Therefore, the 6-month mediator data, rather than the post intervention data, were included in the analysis to reduce the potential for detection bias.…”

“…For example, in contrast to the outcomes reported in the systematic review, several studies conducted amongst the Spanish population with CLBP consistently report no correlation between measures of fear or catastrophizing with pain and disability. [15][16][17] Consequently, while pain education approaches appear to be effective for reducing pain-related disability in liberal and Anglo-Saxon cultures, [18][19][20] a biomedical approach for CLBP has been more effective in Spain's Latin-Mediterranean culture. 21,22 The conflicting evidence suggests that socio-cultural environment influences psychosocial factors; hence, findings confirming the fear-avoidance model in a western liberal society cannot be generalized to other cultures.…”
