Background The burden of cancer in Sri Lanka is on the rise. The overall incidence of cancer in Sri Lanka has doubled over the past 25 years with a parallel rise in cancer-related mortality. Cancer has become the second commonest cause of hospital mortality in Sri Lanka. In this review, we aim to provide an overview of the current status and future direction of cancer care in Sri Lanka. Main body In Sri Lanka, cancer services are predominantly provided by the state sector free of charge to the general public. With the establishment of national cancer policy on cancer prevention and control, there has been a commendable improvement in the cancer services provided island-wide. An increasing number of breast, oropharyngeal, thyroid, oesophageal, colorectal, lung, and gastric cancers are being diagnosed and treated annually. Primary prevention measures include restrictions in tobacco and HPV vaccination. Screening programs for selected cancers such as breast, oral and cervical cancers are delivered. Medical oncology units with facilities for systemic therapy and adequately supported by surgical, pathology, and radiology departments have been established in each district general hospital island-wide. Although the current progress is commendable, future changes are necessary to overcome the current limitations and to cater the ever increasing burden of cancer. Measures are necessary to enhance the coverage of Sri Lanka Cancer Registry. Timely high-quality research and audits are essential. Community participation in planning strategies for cancer prevention and treatment is minimal. Community-based palliative care facilities and radiation and other systemic therapy should be made available in all provinces. A culture of multi-disciplinary care with proper referral pathways would help to improve the current setting. Conclusion In conclusion, Sri Lanka has a reasonably balanced and continuously expanding program for prevention, screening, and treatment of cancers. Emphasis on preventive strategies related to reducing tobacco smoking, chewing betel, and obesity, making cancers a notifiable disease, involving the community in planning cancer care and prevention strategies, conducting research to evaluate cost-effectiveness of existing treatment and increasing radiotherapy facilities would further improve the cancer services in Sri Lanka.

Von Hippel-Lindau [VHL] syndrome is a hereditary multisystem cancer syndrome characterized by a wide range of benign and malignant tumours. Renal neoplasms are the commonest solid organ malignancies associated with VHL seen in 25-45% of patients. Management of renal tumours is determined by multiple tumour related and patient-related factors and decision making is often difficult due to its multifocal nature and bilateral involvement. Its tendency for frequent recurrences further complicates the clinical picture. We review the existing literature on the management of renal malignancies in VHL syndrome focussing on observational strategies, minimally invasive approaches, nephron-sparing surgery and radical surgery. Furthermore, we provide the Sri Lanka perspective in the management of renal malignancies in VHL syndrome. Surgeons that manage such patients should be cognizant of extra-renal manifestations of VHL and the importance of a multidisciplinary team and referral pathways. Proper follow-up and selecting patients for suitable as well as minimal interventions such as minimally invasive techniques and nephron-sparing approaches is crucial in the management to provide acceptable cancer control while preserving renal functions. In Sri Lanka, establishing proper referral pathways to dedicated centres with a multidisciplinary team equipped with facilities to deal with renal as well as extra-renal manifestations would improve expertise, quality of care and reduce patient discomfort and default rate.

Survival Rate of Kidney Cancer in the Asian Countries: A Systematic Review and Meta-Analysis study

Background: Kidney cancer is one of the most deadly cancers of the urinary tract. Among the different continents, Asia has the highest percentage in terms of incidence and mortality. In the present study, we aimed to provide a comprehensive estimate of kidney cancer survival in Asian countries. Methods: The current study is a systematic review and meta-analysis of kidney cancer survival in Asian countries. Researchers in the study searched for articles published in six international databases. We included all observational studies (cross-sectional, case-control, and cohort studies) published up to June 1, 2022. Survival and exclusively localized renal cell carcinoma in Asian countries were included in the study. The Newcastle-Ottawa Quality Assessment Form has been prepared in previous studies to evaluate the quality of papers. The random-effects model was used to reduce the risk of bias in the studies. The Egger bias test was also used to assess the risk of publication bias. Results: In total, 42 articles had criteria for inclusion in the analysis. The one-year survival rate in Asian countries was 71% (95% CI 66.2-75.3). The five-year survival rate was 69.4% (95% CI 67.7-71.0), and the 10-year survival rate was 62.3% (95% CI 57.6-66.7). The meta-regression results showed a significant association between publication year and one-year and five-year survival rates. Thus, the year of study is a cause of variability in results of one-year (Reg Coef = 0.099, P<0.001) and five-year (Reg Coef = 0.045, P<0.001) survival rates. Human Development Index was a cause of variability in results of one-year (Reg Coef = 4.80, P<0.001) and five-year survival rates (Reg Coef = 2.90, P<0.001). Conclusion: The survival rate of kidney cancer in Asian countries is lower than in European countries and the United States.
