Abstract: Improvements in sensor accuracy, greater convenience and ease of use, and expanding reimbursement have led to growing adoption of continuous glucose monitoring (CGM). However, successful utilization of CGM technology in routine clinical practice remains relatively low. This may be due in part to the lack of clear and agreed-upon glycemic targets that both diabetes teams and people with diabetes can work toward. Although unified recommendations for use of key CGM metrics have been established in three separate … Show more

“…Indeed, over the past year TIR has become an attractive metric in both clinical and academic fields as it combines mean glucose and a measure of variability into a single measure. A recent consensus statement outlined target TIR values and time‐below‐range values in different conditions such as pregnancy and T1DM . A recent re‐analysis of seven‐point capillary glucose profiles from the DCCT validated TIR as a marker of microvascular risk and showed that, for every 10% reduction in TIR, there was a 64% increase in risk of retinopathy and a 40% increase in risk of microalbuminuria .…”

“…As we begin to understand the variability in glucose, the aim of therapy is shifting towards reducing extreme excursions and maximizing TIR. Recent recommendations from the International Consensus on Time in Range lay out target values for time in and below range in different age groups, recommending a target of 70% TIR for most individuals with insulin‐treated diabetes, and aiming for under 4% of time below 70 mg/dL …”

“…In between visits, she uploaded her pump and CGM to CareLink® Clinical Software weekly and received recommendations for pump adjustments. The time in range for CareLink® Clinical Software reports cannot be changed, thus we could not generate reports using the newly recommended pregnancy time in range of 63 to 140 mg/dL (3.5‐7.8 mmol/L) . Therefore, the data in this case series are presented per standard recommendations for non‐pregnant adults.…”

“…A glucose target in HCL systems that is lower than 120 mg/dL (6.7 mmol/L) would be preferable for pregnancy. Finally, it is important to note that the time in range for CareLink® Clinical Software reports cannot be changed to the newly recommended pregnancy time in range of 63 to 140 mg/dL (3.5‐7.8 mmol/L) . This makes it more difficult to compare results of other systems previously studied in pregnancies to the 670G system case series we present here and it is a limitation of using the 670G system during pregnancy.…”

“…The downside of this though was losing my hypo warning signs, as a result of experiencing too many hypos. I ended up chasing my tail, with many swings during the day, and was definitely not experiencing the recommended periods of 'time in range' [3]. This impacted negatively on work, my Correspondence to: Bridget Turner.…”
