Clinical Evidence behind Stereotactic Radiotherapy for the Treatment of Ventricular Tachycardia (STAR)—A Comprehensive Review

Abstract: The electrophysiology-guided noninvasive cardiac radioablation, also known as STAR (stereotactic arrhythmia radioablation) is an emerging treatment method for persistent ventricular tachycardia. Since its first application in 2012 in Stanford Cancer Institute, and a year later in University Hospital Ostrava, Czech Republic, the authors from all around the world have published case reports and case series, and several prospective trials were established. In this article, we would like to discuss the available c… Show more

“…However, to date the entire literature describes less than 60 patients treated by this novel approach. [11][12][13] Recognising the urgent need for more effective treatments in patients with refractory VT, three UK tertiary cardiac centres agreed to collaborate in developing cardiac SABR services aiming to maximise the benefits of combined experience of a novel technique. We now report our initial experiences.…”

“…It has been shown to reduce the VT burden in patients who are difficult to treat with medication and/or catheter ablation or cannot tolerate catheter ablation [75]. However, there are significant differences in this emerging treatment between studies including a significant lack of consistency in the treatment protocols [76]. Therefore, more RCTs are still needed to establish a detailed treatment strategy.…”

“…To no surprise, the first case reports on STAR ( 4 6 ) and encouraging results of the phase I/II study by Robinson et al ( 7 ) have met with avid interest, resulting in multiple centers adopting the method as compassionate treatment and within new prospective clinical trials ( 8 ). One of them is the Polish trial SMART-VT ( ClinicalTrials.gov Identifier: NCT04642963) ( 9 ).…”
