Abstract: Connective tissue graft under CAF results in increased probability of CRC only at sites with thin baseline gingiva. CAF alone is associated with similar clinical outcomes and better aesthetics at sites with thick baseline gingiva.

“…Bednarz et al reported that fascia lata allograft in conjunction with MCAT technique produced a comparable mean root coverage of 94% to SCTG after 6 months [12]. In four out of five studies where coronally positioned flaps (CPF) alone was compared to SCTG, significantly better root coverage was achieved with the graft [14,15,[21][22][23]. Cairo et al confirmed this and showed that the treatment results of the MCAF alone vs. MCAF plus SCTG procedures were similar at sites with thick gingiva (>0.8 mm) but the SCTG improved results at sites with thin tissue [14].…”

In two studies, acellular dermal matrix allograft (ADM, AlloDerm®, BioHorizons) reduced gingival recession by at least 90% and showed no statistically significant difference from SCTG groups [15,27].…”

“…9 On the other hand, CTG acts as a biological filler to enhance the flap adaptation to the root surface and to increase gingival thickness. 10 In a systematic review and meta-analysis, Thoma et al concluded that autogenous grafts (either CTG or FGGT) were the most effective technique in augmenting keratinized mucosa at implant sites, 7 a crucial factor for maintaining peri-implant health. 11 Clinicians have explored different approaches for harvesting a CTG from the palate, while aiming at minimizing patient morbidity and complications, including intra-surgical and post-surgical excessive bleeding, flap dehiscence and donor site infections.…”
