Introduction:Vaginal agenesis may present as a single defect in development or associated with other anomalies. It is usually associated with Mayer-Rokitansky-Kuster-Hauser (MRKH) and Androgen Insensitivity Syndromes (AIS). The incidence is about 1 in 4000-5000 live female births. This is a case series of six cases of vaginoplasties performed by three varied surgical techniques over a period of two years, with a follow up. Cases: A comparison of three techniques of vaginoplasties was done. Six total cases were included-three cases using modified McIndoe's technique with amnion graft, two using split thickness skin graft and one by Laparoscopic Peritoneal Pull through technique. Patients were followed up meticulously over the course of two years and results were recorded. The comparison scoring was done based on relative ease of the operative procedure, pain experienced, vaginal depth on follow up, uptake of graft and sexual satisfaction in the married females. Result: Despite new techniques of vaginoplasty advancing over the years such as laparoscopic approaches and use of different materials as grafts, vaginoplasty with amnion graft is stillpreferredfor the treatment of vaginal agenesis.The technique is simple and safe and provides a satisfactory and functional vagina in majority of the patients.

Objective: To critically appraise the literature exploring the surgical treatment of degenerative lumbosacral stenosis in dogs. Background: Several surgical procedures to treat degenerative lumbosacral stenosis (DLSS) in the dog have been reported, however, definitive criteria for surgical technique preference are currently lacking (1). Evidentiary value and methods: A critical appraisal tool that examined the conduct and reporting of studies on the results of surgical treatment of DLSS was designed and, after a systematic search and screening of the literature, a critical appraisal was performed in 20 papers. Results: Results showed that 18/20 studies included did not clearly report inclusion and exclusion criteria and in 14/20 it was unclear whether consecutive inclusion of participants was applied. 19/20 studies reported age, breed, and sex of the participants, and 13/20 did not report duration and prevalence of clinical signs. In 13/20 studies, it was considered that the condition was not measured in a standard and reliable way. Objective outcome measures were used in 7/20 studies. Conclusion and application: The results demonstrate that there is room for improvement in the conduct and reporting quality of case series so that rigorous data can be generated and analysed, to inform research design, guide clinical practice, and improve veterinary healthcare delivery.

Background Root cause analysis is a technique used to assess systems factors related to “sentinel events” –serious adverse events within healthcare systems. This technique is commonly used to identify factors, which allowed these adverse events to occur, to target areas for improvement and to improve health care delivery systems. We sought to apply this technique to men presenting with metastatic prostate cancer. Methods We performed an in-depth case series analysis of 15 patients who presented with metastatic disease at Johns Hopkins Sidney Kimmel Comprehensive Cancer Center using root cause analysis to refine a list of health system factors that lead to late stage presentation in the current era. Results Key factors in late diagnosis of prostate cancer included lack of insurance, lack of routine PSA testing, comorbidities, reticence of patients to follow up actionable PSA, and aggressive disease. Three patients had aggressive disease that would not have been discovered at an early stage in the disease process, despite routine screening. However, analysis of the remaining 12 patients illuminated health system factors led to missing important diagnostic information, which might have led to diagnosis of PCa at a curable stage. Conclusions The cases help highlight the need for systems based approaches to early diagnosis of prostate cancer. A heterogeneous group of barriers to early diagnosis were identified in our series of patients including economic, health systems and cultural factors. These findings underscore the need for individualized approaches to preventing delayed diagnosis of prostate cancer. While limited by our single-institution scope, this approach provides a model for research and quality improvement initiatives to identify modifiable systems factors impeding appropriate diagnoses of prostate cancer.
