Clicking Eyelids: A New Finding of Prostaglandin-Associated Periorbitopathy

Abstract: Prostaglandin-associated periorbitopathy and the resulting cosmetic effects noticed visually by patients are well documented. Our case represents a new audible sign and symptom that may be found with prostaglandin-associated periorbitopathy but has not to our knowledge been reported in the literature.

“…Most articles described an association between eyelash trichomegaly and the prostaglandin analogue, bimatoprost, the only Food and Drug Administration approved medication for eyelash growth in the United States (US) 37 . Several articles reported eyelash lengthening as an adverse event (AE) of bimatoprost for treatment of glaucoma 38–43 . Others specifically described the effect of bimatoprost on inducing eyelash trichomegaly.…”

“…It is the first time we encounter the “click” sound on the blinking in a CS case, possibly due to loss of periorbital fat tissue. Audible clicking blinks have also been scarcely reported in ophthalmology case reports, related to prostaglandin‐associated periorbitopathy with sunken eyes and deepened upper eyelid sulci and these reports hypothesized that periorbital fat atrophy caused by prostaglandin medications was responsible from the clicking sound 13–15 . It has not been described before in CS, and therefore does not have a clear diagnostic value but it may help clinicians in future evaluation of adult‐onset CS.…”

“…Its onset is insidious and is frequently overlooked. PAP can be symptomatic in some patients, even producing an audible clicking sound upon blinking [63,64]. Asymptomatic cases can become problematic at the time of ocular surgery, in that enophthalmos associated with PAP can increase the complexity of intraocular surgery, as well as having implications for patients undergoing oculoplastic surgery.…”
