A 23-year-old Japanese woman with a cutaneous ciliated cyst on her right lower leg is reported. A subcutaneous cyst, measuring 2.5 cm in diameter with papillary projections into the lumen, was lined with ciliated cuboidal to columnar epithelia with partial stratification, histologically. These lining cells did not produce mucin. Immunohistochemically, the ciliated lining cells of the cyst were diffusely positive to epithelial membrane antigen and cytokeratin. In addition, positive immunoreaction with anti-desmin monoclonal antibody was observed in the body of the cilia. Less than 10% of the epithelial cells revealed positive immunoreaction to S-100 protein and estrogen receptor.

Vulval swellings have always been a case of a difficult and a puzzling situation and more so when they are huge in size. Vulvar swellings are of various types such as Bartholin cyst, sebaceous cyst, cyst of canal of nuck, inguino-labial hernia and vulval varicosities. Most common vulvar cysts are epidermal inclusion cysts. Usual location is beneath the epidermis. An alternative histogenesis is embryonic remnants or occlusion of pilosebaceous ducts of sweat glands. Cutaneous cysts which are lined by ciliated epithelium are very rare, and authors present a rare case of a cyst arising from a left labium majus resembling a hydrocele seen in males with histopathology suggestive of ciliated cyst of the vulva.

We report a 36-year-old woman with a cutaneous cyst of the dorsal aspect of the left foot. Because of the symptoms of pain and paresthesias attributed to the lesion, it was clinically felt to be a ganglion. After excision, these symptoms abated. Yet histologic examination showed this lesion to be a rare tumor, the ciliated cutaneous cyst. This is the third patient with this distinctive lesion on the foot or ankle.
