Chronic interstitial lung disease in children: Response to high‐dose intravenous methylprednisolone pulses

Abstract: Summary. The prognosis for children with chronic interstitial lung disease is poor and the mortality rate is high, especially in infants. This explains the many therapeutical protocols which have been proposed and investigated by several authors. In the present work, we evaluated the response of three infants with idiopathic pulmonary fibrosis to high-dose intravenous prednisolone pulses. The patients were referred to the department at the age of 4, 17, and 3 months, respectively. The diagnosis was confirmed by… Show more

“…HDPIM therapy was introduced over 30 years ago in the treatment of renal grafts,28 and has since been expanded to treat a variety of immune‐mediated inflammatory diseases especially rheumatologic ones 29. The use of HDPIM in pediatric pulmonology was first described in three infants with severe idiopathic interstitial fibrosis in 19989 resulting in improvement with no side effects including adrenal insufficiency, which is frequently seen after treatment with oral prednisolone. We have used HDPIM as basic regimen in pediatric HP with the same few observations of side effects.…”

“…From experience with rheumatoid disease, the maximum effect is achieved after 2 weeks. Recently, a series of case reports showed that pulse therapy was effective in interstitial lung diseases in children unresponsive to continuous systemic steroid therapy (9,10). The efficacy was found to be superior to continuous systemic treatments in some reports.…”

“…HDPM treatment is recommended in other inflammatory lung diseases in children. 75,76 Reported side effects include transient flushing, headache, mood changes 77 and sinus bradycardia, 78 but no serious long-term events have been described in children. HDPM has been the standard treatment of BO since 1999 at our centre, and we here summarise the results.…”
