Chiari pelvic osteotomy for osteoarthritis secondary to hip dysplasia. Indications and long-term results

Abstract: Of 142 Chiari pelvic osteotomies for osteoarthritis in dysplastic hips, most performed by Chiari himself, we were able to review 82 and obtain information about 18 by questionnaire. All patients were over 30 years of age at operation; follow-up averaged 15.5 years. Twenty hips had undergone secondary total hip replacement. The outcome was good in 75%, fair in 9% and poor in 16%. High osteotomies all gave good results, and the result also depended on adequate medialisation. Statistics were worse for patients ov… Show more

“…Chiari and others have considered the degree of medial displacement to be important. 2,6,11 We achieved medial displacement of 20% to 50% in most cases, but were unable to determine an ideal degree. We observed a reduction in the percentage of cases with progressive change when the postoperative CE angle was 30° to 60° or the medial displacement 40% to 60%, but this was not statistically significant.…”

“…Previously, osteotomy was recommended mostly for younger persons, with the mean age in most earlier osteotomy series between 20 and 40 years [5,12,15,20,23]. Ninomiya and Tagawa [20] did not recommend osteotomy in elderly patients because of limping owing to weakness of the gluteus medius.…”
