Characterization of drivers maintaining atrial fibrillation: Correlation with markers of rapidity and organization on spectral analysis

Abstract: Intermittent focal or rotational drivers were identified in all patients. Drivers consistently correlated to organization markers. Greater temporal stability and organization predicted AF termination with driver ablation.

“…The definition used for localized drivers in AF continues to be debated with alternative definitions used in different studies . There are data emerging from centers using different mapping technologies suggesting that AF may be sustained by focal and rotational sources that are spatially stable but with temporal periodicity . However, it remains unclear how best to map and ablate these potential drivers.…”

“…Ablation was stopped if: (a) 5 minutes of ablation had been performed, (b) no signal remained at the ablation site, or (c) a study‐defined ablation response had been achieved. Ablation responses were defined as the termination of AF (into AT or sinus rhythm), or CL slowing of ≥30 ms. Our group has previously applied this definition . AF CL was measured over 30 beats from the PentaRay catheter positioned in the LA appendage.…”

“…CARTO has recently produced CARTOFINDER (Biosense Webster) which times electrograms relative to each other in a 250 ms window which then moves through a continuous recording to show wavefront movement over time. This has enjoyed some success demonstrating focal and rotational activations in AF and an automated approach to their detection has been developed . The Topera and ECGi mapping systems rely on phase mapping to identify drivers.…”

“…The maximum GEPD was varied between 1 and 9 cm for all STAR maps. For each GEPD the number of paired electrodes was manually determined from all the maps for each anatomical LA surface using a previously described LA model . In brief, the LA was segmented into five anatomical surfaces (anterior, roof, septum, lateral, and posterior‐inferior).…”
