Background: Chronic knee pain from advanced osteoarthritis is often associated with tears in the fibro-cartilaginous meniscus and cruciate ligaments. The result is substantial pain and impairments and loss of daily function, thus limiting activity and quality of life. These often include pain, loss of range of 116 motion, inability to elevate the leg, pain increased with activity and gradual worsening over time. This represents a challenging clinical picture for which few non-surgical options are available. Most patients failing to respond to current methods of care are offered arthroplasty. Current therapies are focused on the relief of the symptoms but cannot repair the damage nor stop the degenerative process. Objective: This is a case study of the first subject in a continuing IRB to reach a 2.5 year end-point. Our intention was to investigate the potential benefits of using autologous, minimally manipulated fat graft in subjects with advanced osteoarthritis and meniscus pathology who had failed to obtain benefit from previous, standards of care. Study description: 73 YO male, end stage bone-on-bone right knee pain. First subject to reach the 2 1/2 year follow up after Lipogems® precision ultrasound guided injections. Material and Methods: Micro-fragmented fat was obtained using a minimal manipulation technology in a closed system, Lipogems®, without the addition of enzymes or other additives. Fat graft was injected under continuous ultrasound guidance. Results: VAS pain, KOOS function, range of motion and quadriceps strength improved after treatment. Conclusion: Autologous, micronized, and minimally manipulated adipose tissue resulted in significant improvement in pain, function and quality of life. No adverse events were reported.

INTRODUZIONEIl processo di guarigione atteso nella ferita acuta si interrompe nelle ulcere cutanee arrivando così ad una cronicizzazione del letto di ferita. Pur in un quadro di approccio olistico e globale, si deve tuttavia intervenire localmente determinando una modifica dell'ambiente tale da favorire la riepitelizzazione.1 Con l'aumento della patologia una serie di centri specialistici e dedicati stanno trovando una progressiva diffusione sul panorama italiano. Questi centri spesso tendono a sperimentare nuovi presidi, proprio in quanto a essi affluisce una popolazione di pazienti sempre più complessa e difficile. Abbiamo analizzato la nostra esperienza nell'uso delle cellule staminali mesenchimali estratte dal tessuto adiposo attraverso un sistema dedicato.Il sistema Lipogems® è un sistema di lipoaspirazione, processazione e innesto di tessuto adiposo.2 L'intera procedura avviene in un unico tempo chirurgico in anestesia generale o epidurale. Si aspirano circa 60 cc di tessuto adiposo dal grasso comunemente presente a livello dell'addome o della coscia. Il tessuto così prelevato viene introdotto in un sistema chiuso e asettico. Attraverso una minima processazione di tipo meccanico enzyme free si ottiene un tessuto adiposo microfratturato non espanso de- RIASSUNTOLa guarigione di una ferita cronica è un processo articolato e complesso, che richiede una visione clinica globale del paziente. Tuttavia, il trattamento locale del letto della ferita riveste un'importanza fondamentale. Sempre più ormai si parla di medicina rigenerativa di tipo induttivo come mezzo per stimolare le cellule senescenti all'interno di una lesione cutanea cronica, in particolare mediante l'utilizzo delle cellule staminali mesenchimali. Il tessuto adiposo autologo rappresenta una fonte di cellule staminali e la tecnologia Lipogems® ne rende possibile la sua applicazione clinica. In questo studio vengono analizzati i risultati ottenuti con tale metodica che, seppur con i limiti di un numero ancora non consistente di pazienti, sono comunque incoraggianti nel proseguire lungo questa strada, considerata da molti come l'ultima frontiera nel wound care. Since 2005, sex determining region y-box 2 (SOX2) has drawn the attention of the scientific community for being one of the key transcription factors responsible for pluripotency induction in somatic stem cells. Our research investigated the turnover regulation of SOX2 mRNA in human adipose-derived stem cells, considered one of the most valuable sources of somatic stem cells in regenerative medicine. Mitoxantrone is a drug that acts on nucleic acids primarily used to treat certain types of cancer and was recently shown to ameliorate the outcome of autoimmune diseases such as multiple sclerosis. In addition, mitoxantrone has been shown to inhibit the binding of human antigen R (HuR) RNA-binding protein to tumor necrosis factor-a mRNA. Our results show that HuR binds to the 39-untranslated region of SOX2 mRNA together with the RNAinduced silencing complex miR145. The HuR binding works by stabilizing the interaction between the 39-untranslated region and the RNA-induced silencing complex. Cell exposure to mitoxantrone leads to HuR detachment and the subsequent prolongation of the SOX2 mRNA half-life. The prolonged SOX2 half-life allows improvement of the spheroid-forming capability of the adipose-derived stem cells. The silencing of HuR confirmed the above observations and illustrates how the RNA-binding protein HuR may be a required molecule for regulation of SOX2 mRNA decay.
