We have recently shown that in vitro polarized M-CSF-driven anti-inflammatory macrophages (MU2) have the unique capacity to preferentially bind and ingest early apoptotic cells. However, these data are based on in vitro polarized cells and it is unclear whether MU2-like cells exist in vivo. Here we used CD163 as a cell surface marker to distinguish MU2 from the pro-inflammatory MU1. We show that human peritoneal MU (pMU) freshly isolated from patients on peritoneal dialysis have the phenotypical characteristics of MU2, including CD163 surface expression and lack of CD16. Like MU2, pMU have the capacity for endocytosis and macropinocytosis, are able to preferentially bind and ingest early apoptotic cells, and produce large amounts of IL-10 upon stimulation with LPS. Moreover, upon LPS stimulation both pMU and MU2 downregulate CD86, resulting in a reduced capacity to stimulate proliferation of allogeneic T cells and an inhibition of Th1 cytokine release of these T cells. Our data provide the evidence for the first time that in vitro polarized MU2 exist in vivo, and human pMU resemble the anti-inflammatory MU2. We propose that pMU have the potential to maintain an anti-inflammatory condition in the peritoneal cavity.

Significant alterations of serum cytokine levels in patients with Peyronie's disease

Objective: To determine the expression of the cytokines transforming growth factor-β1 (TGF-β1), interferon-γ (IFN-γ), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) in serum from patients with Peyronie's disease (PD) compared to healthy controls. Materials and Methods: Ninety-one consecutive PD patients aged 20 -74 years were included in this study. All patients were diagnosed with symptomatic PD for the first time and had a palpable penile plaque. The patients previously had the disease for 6 -72 months. None of the patients had a severe infectious disease or known systemic illness. For cytokine analyses, peripheral venous blood samples were obtained before treatment. Fifty healthy male blood donors aged 22 -64 years served as the control group. TGF-β1, IFN-γ, Il-6, and TNF-α were analyzed quantitatively with commercial immunoassays. Results: Mean cytokine levels in serum from patients were increased for TGF-β1 and IFN-γ compared to healthy controls. The difference for TGF-β1 was considered statistically significant (p < 0.001). IL-6 was not detectable in PD patients (p < 0.01) and TNF-α was decreased (p < 0.0001). Conclusion:The significantly elevated serum level of the profibrotic TGF-β1 cytokine underscores the effect of cytokines in the pathophysiology of PD. The significantly decreased TNF-α serum level suggested no acute immunomodulatory process. Therefore, the relevance for therapeutic administration of TNF-α should be further investigated. Quantification of TGF-β1 in serum of PD patients provides a possible diagnostic tool and target for therapy. The data on altered cytokine levels in PD patients also provide a new understanding for etiopathogenesis of PD, which warrants further investigation.

Özet: Bu çalışmada köpeklerde çift taraflı üreter tıkanmasında klinik ve bazı hematolojik bulguların belirlenmesi ve kanda miktarı artan metabolitlerin vücuttan uzaklaştırılması ve kanın biyokimyasal dengesinin korunmasında peritoneal diyaliz uygulamalarının etkinliğinin araştırılması amaçlandı. Paramedian olarak karın boşluğuna girilip, üreterler klipsler ile geçici olarak tıkandı ve parsiyel omentektomi eşliğinde kalıcı Tenckhoff kateteri yerleştirildi. Operasyon sonrası 3 saat aralıklarla kan üre nitrojen (BUN), kreatinin değerleri kontrol edildi ve BUN değeri 90 mg/dl'yi aştığında diyalize başlandı. Diyaliz için %1.5'luk glikoz içeren ticari solüsyon 6 saat arayla 2 gün süresince toplam 8 seans kullanıldı. Klinik kontroller çalışma süresince; kan sayımı sonuçları operasyon öncesi (0) ve sonrasındaki her 24 saatte ve serum parametreleri 0 ve diyalize başladıktan sonra her seansın öncesi ve sonrasında belirlendi. Depresyon, halsizlik, çevreye karşı ilgisizlik, kusma, dehidrasyon ve mukozalarda solgunluk ortak klinik bulgular olarak belirlendi. BUN değeri 90 mg/dl'nin üzerine 30-39. saatlerde çıktı. Operasyon öncesi ile karşılaştırıldığında diyaliz başlangıcında BUN, kreatinin, K + ve P -2 değerleri önemli oranda yükselirken, Total protein (TP) ve albumin değerleri düştü, Na + , Ca +2 ve Cl -değer-leri değişmedi. Diyalize başlandıktan sonra, her seansın sonunda saptanan BUN, kreatinin ve K + değerlerinin seans öncesine göre önemli oranda düştüğü, ancak 0 değerine göre hala yüksek olduğu belirlendi. Diyaliz seansları, TP, albumin ve P -2 değerlerinde de düşmeye neden oldu. Son seans sonrasında BUN, kreatinin ve P -2 değerinin operasyon öncesi değere göre yüksek, TP, albumin ve Cldeğerinin ise düşük olduğu belirlendi. Sonuç olarak, bilateral tam üreteral obstrüksiyon oluşturulan köpeklerde 6 saat ara ile yapılan her diyaliz seansının vücuttaki zararlı metabolitlerin uzaklaştırılmasında etkili olduğu ancak sağlıklı hayvanlardaki düzeylere kadar düşüremediği ve uygulamalar sırasında hipoalbuminemi ile karşılaşılabileceği, kalıcı Tenckhoff kateterin köpeklerde parsiyel omentektomi eşliğinde güvenle kullanılabileceği kanısına varıldı.Anahtar sözcükler: Köpek, peritoneal diyaliz, üreteral obstrüksiyon Peritoneal dialysis applications in experimentally induced bilateral ureteral obstruction in dogsSummary: This study was designed to determine clinical and some hematological changes associated with bilateral uretheral obstruction and effect of peritoneal dialysis in discard of increased metabolites in blood and in maintanence of biochemical balance of blood in dogs.
