Abstract: Septic multi-organ failure represents a common cause for operative mortality following open-heart surgery. One major reason might be the depression of cell-mediated immunity. The purpose of this prospective randomized trial was to quantify and specify the effects of open-heart surgery on cell-mediated immune mechanisms. In addition, the immunorestorative potential of a combined immunomodulatory therapy with the cyclooxygenase inhibitor indomethacin and the thymomimetic substance Thymopentin versus single drug … Show more

“…Most previous studies showed recovery of cell-mediated immunity 7 days after cardiac surgery. Markewizt et al, however, showed significantly suppressed responses to an antigen-cocktail or PHA 7 and a delayed hypersensitivity skin reaction 15 7 days after cardiac surgery. Response to the antigen cocktail decreased to 53.4% on POD 1 recovering to 72.3% on POD 7 -findings both much higher than in our report.…”

“…1,2,4,6,7,10,11 Since none of these indicators is antigenspecific, they do not always reflect adaptive immunity to a particular pathogen. It remains unclear, therefore, whether adaptive immunity is affected by open-heart surgery.…”

“…First, after cardiac surgery, the number of T lymphocytes in peripheral blood is reduced [27,28]. In our study, the hemodilution-induced reduction played only a minor role because we adjusted the PBMCs to a predetermined concentration.…”

“…1 An open heart surgery, which is associated with the usage of CPB, has been reported to cause an in vitro temporary cellular immune depression, which is most pronounced in the first 2 days after surgery, may last for about 2 weeks, and is manifested by an in vitro attenuated lymphocytes response to mitogens, as well as change in T cell subpopulations. [4][5][6][7][8][9] Sano et al, 4 examined lymphocyte proliferation in vitro, after incubation with PPD antigen, in adults undergoing open heart surgery. They found that in the first 2 days after surgery the amount of lymphocytes proliferation was only 3.2% of the presurgical response.…”
