Bone strength is determined by bone mineral density (BMD) and bone quality. Bone fractures are serious adverse events in men receiving androgen deprivation therapy (ADT) for prostate cancer. Although it is established that ADT reduces BMD, it is unclear how ADT alters bone quality. Vitamin K is a bone quality marker which aids to carboxylates osteocalcin to form bone matrix. Serum undercarboxylated osteocalcin (ucOC) is accumulated in vitamin K deficiency, which is used as a surrogate for vitamin K status. As little is known as to the impact of ADT on serum ucOC, we investigated the changes of serum ucOC in PC patients during ADT. Fifty consecutive hormone naïve PC patients were enrolled. Serum ucOC, serum osteocalcin (OC), serum Nteloptide of type 1 collagen (NTx) and hip bone mineral density (BMD) were measured at baseline, 6 month and 12 months since the start of ADT. Serum ucOC levels at 6 and 12 months (3.86 ± 2.28 and 4.32 ± 1.76 ng/ml) were significantly higher than those at baseline (2.46 ± 1.46 ng/ml). Serum OC levels at 12 months (7.82 ± 2.65 ng/ml) were significantly higher than those at baseline (5.26 ± 1.86 ng/ml). Furthermore, the ratio of ucOC/OC at 1 year (0.54 ± 0.15) were significantly higher than at baseline (0.42 ± 0.18). Both serum NTx levels and hip BMDs at 6 and 12 months were also significantly higher than those at baseline. Our results first demonstrated that ADT induced vitamin K loss in men with prostate cancer. Further investigations are required to determine whether vitamin K supplement is useful to prevent bone fractures in men on ADT.

Background Oophorectomy is recommended for women at increased risk for ovarian cancer. When performed at premenopausal age oophorectomy induces acute surgical menopause, with unwanted consequences. Objective To investigate bone mineral density (BMD) and fracture prevalence after surgical menopause. Search strategy A literature search of PubMed, EMBASE and Cochrane library was performed with no date restriction. Date of last search was March 1st, 2016. Selection criteria Primary studies reporting on BMD, T‐scores or fracture prevalence in women with surgical menopause and age‐matched control groups. Data collection and analysis Data were extracted on BMD (g/cm2), T‐scores and fracture prevalence in women with surgical menopause and control groups. Quality was assessed by an adaptation of the Downs and Black checklist. Random effects models were used to meta‐analyse results of studies reporting on BMD or fracture rates. Main results Seventeen studies were included, comprising 43 386 women with surgical menopause. Ten studies provided sufficient data for meta‐analysis. BMD after surgical menopause was significantly lower than in premenopausal age‐matched women [mean difference lumbar spine, −0.15 g/cm2 (95% CI, −0.19 to −0.11 g/cm2); femoral neck, −0.17 g/cm2 (95% CI, −0.23 to −0.11 g/cm2)] but not lower than in women with natural menopause [lumbar spine, −0.02 g/cm2 (95% CI, −0.04 to 0.00 g/cm2); femoral neck, 0.04 g/cm2 (95% CI, −0.09 to 0.16 g/cm2)]. Hip fracture rate was not higher after surgical menopause compared with natural menopause [hazard ratio: 0.85 (95% CI, 0.70 to 1.04)]. Author's conclusions No evident effect of surgical menopause was observed on BMD and fracture prevalence compared with natural menopause. However, available studies are prone to bias and need to be interpreted with caution. Uncarboxylated osteocalcin (ucOC) is important in evaluating vitamin K status and it is inversely associated with bone mineral density (BMD). We studied the correlationship between ucOC and BMD in healthy Korean women. This study recruited 337 healthy women between ages 20-70 were recruited. Serum ucOC, calcium, alkaline phosphatase, body mass index (BMI), and BMD were measured and compared. Mean BMI was lowest (20.3±1.9 kg/m2) in the 20 yr old group and highest (24.8±2.6 kg/m2) in the 60 yr old group. Women age 20-70 yr old had ucOC inversely related to BMD independent of other factors that may influence BMD. Serum ucOC concentration and BMD of lumbar spine showed a significant inverse relationship. Serum mean alkaline phosphatase was lowest (122±30 IU/L) in the age 30 group and highest (190.3±55.8 IU/L) in the age 60 group. Serum ucOC was inversely associated with BMI, and positively associated with alkaline phosphatase. Uncarboxylated osteocalcin (ucOC) was inversely associated with spinal BMD in healthy Korean women. Serum mean ucOC was highest in the age 20 group, followed by age 50 group, which may indicate vitamin K insufficiency could be related to high bone turnover in these groups. These results suggest that vitamin K supplement may be considered to help both bone growth and bone loss during these periods.
