Background: No candidate biomarkers based on cerebrospinal fluid (CSF) have been identified as prognostic factors in patients with major depression treated with electroconvulsive therapy (ECT), yet. Method: Following different underlying hypotheses, we analysed baseline CSF levels of markers of neurodegeneration (tau proteins, β-amyloids and neurogranin), elements of the innate immune system (interleukin [IL]-6, neopterin, soluble CD14, soluble CD163, migration inhibitory factor and monocyte chemotactic protein 1), endocannabinoids, sphingolipids and Klotho before ECT in patients with depression (n = 12) to identify possible correlations with the clinical antidepressant response to ECT. Results: Correlation with the reduction of the depressive symptoms could be observed especially for markers of neurodegeneration and elements of the innate immune system. Differences for CSF levels of several markers were found between the groups of responders and non-responders at the trend level. Limitations: The sample size is small and the distribution of responders and non-responders is uneven. Conclusions: It is this first study on CSF biomarkers for antidepressant efficacy of ECT warrants further research regarding the mechanism of ECT and personalized antidepressant therapy.

Ο εγκέφαλος είναι το όργανο με τη μεγαλύτερη περιεκτικότητα σε λίπος. Τα λιπίδια της κυτταρικής μεμβράνης επηρεάζουν τη θέση και τη λειτουργία των μεμβρανικών πρωτεϊνών και με αυτόν τον τρόπο ρυθμίζουν διάφορες λειτουργίες τους. Επομένως μεταβολές στη λιπιδική σύσταση της κυτταρικής μεμβράνης μπορεί να τροποποιούν το μικροπεριβάλλον και άρα λειτουργίες των πρωτεϊνών, όπως η νευρομεταβίβαση. Μερικές από τις πρώτες μελέτες αναφέρονται στην αρνητική συσχέτιση των επιπέδων χοληστερόλης ορού με κατάθλιψη, αυτοκτονικότητα και διαταραχές συμπεριφοράς. Αρκετές μελέτες -αλλά όχι όλες -αναφέρουν μείωση των επιπέδων των ω-3-πολυακόρεστων λιπαρών οξέων (ω-3-PUFA) στο πλάσμα ή τη μεμβράνη των ερυθροκυττάρων σε ασθενείς με κατάθλιψη, διπολική διαταραχή ή μετά από απόπειρα αυτοκτονίας, ενώ έχουν αναφερθεί σε ορισμένες περιπτώσεις θετικά αποτελέσματα μετά από χορήγησή τους. Ο ρόλος των ω-3-PUFA στις συναισθηματικές διαταραχές αποδίδεται στις δράσεις τους στη νευρομεταβίβαση και τη νευροπλαστικότητα καθώς και τις αντιφλεγμονώδεις και αντιοξειδωτικές τους ιδιότητες. Μειωμένα επίπεδα ω-3-PUFA έχουν βρεθεί στα ερυθροκύτταρα ή τα αιμοπετάλια σχιζοφρενών ασθενών. Μελέτες έδειξαν αυξημένο ρυθμό αποδόμησης των φωσφολιπιδίων και μειωμένη αναλογία ενσωμάτωσης ω-3-PUFA στα φωσφολιπίδια σχιζοφρενών, πιθανώς λόγω αυξημένης δραστηριότητας της φωσφολιπάσης Α 2 , ενός ενζύμου με σημαντικό ρόλο στις διεργασίες μετάδοσης του σήματος. Διατροφή ανεπαρκής σε ω-3 PUFA ενδέχεται να αυξάνει τον κίνδυνο εμφάνισης σχιζοφρένειας, ενώ δίαιτα υψηλής περιεκτικότητας σε αυτά μπορεί να έχει προστατευτικό ρόλο έναντι της νόσου και θετική επίδραση στην πορεία της. Αν και η αξία της χορήγησής τους ως μονοθεραπείας δεν έχει καταδειχθεί, μελετάται η χρησιμότητά τους ως θεραπεία ενίσχυσης της φαρμακευτικής αγωγής. Ορισμένοι συγγραφείς θεωρούν ότι δυσλειτουργία της μεταβολικής οδού των σφιγγολιπιδίων, που οδηγεί σε συσσώρευση κεραμιδίων, μπορεί να ευθύνεται για διαταραχές της διάθεσης και αγχώδεις διαταραχές καθώς επίσης και
