The objective of this study was to evaluate the cytological content of ameloblastomas of the jaw. Nine cases of ameloblastoma were punctured, and the intralesional material was processed using the cell block technique. After centrifugation, the pellet obtained from the punctured material was fixed in formaldehyde and routinely processed to inclusion in paraffin. The obtained sections were stained with haematoxylin and eosin (H&E). Immunohistochemical reactions against anti-pan-cytokeratin (AE1/AE3) were performed to measure the presence of epithelial cells. Cytological analyses of the obtained slides revealed the presence of epithelial cells (as evidenced by AE1/ AE3 labelling) and acellular amorphous eosinophilic materials. These cytological findings, in light of clinical and imaging data, can be helpful in the presumptive diagnosis of this disease entity by eliminating other possible diagnoses. Nonetheless further studies are needed in order to determine the nature of the acellular amorphous eosinophilic material and to ascertain the immunoprofile of epithelial cells.

Background: Cystic fluids encountered during routine FNA poses a diagnostic challenge to cytopathologists due to its low cell yield with high dispersal of cells on conventional centrifuged smears (CS). Cell Block (CB) technique enables retrieval of small tissue fragments from fluids, thereby providing scope for better morphology and material for ancillary techniques which help in improving the diagnosis. Aim:To compare the efficacy of CB versus CS, in cyto-diagnosis of cystic lesions. Methods:This observational study was conducted on a total of 50 fluid samples aspirated from cystic lesions during routine FNA and fluids aspirated peri-operatively from cystic ovarian lesions. Divided into two equal parts, one part was processed for CS and CB by Fixed Sediment Method and relevant immunohistochemistry was performed. CSs were categorized as positive for malignancy, benign diagnosis, Inadequate for opinion and suspicious for malignancy. CBs were categorized as; no material, Non-contributory (CS+, CB-), confirms the smear diagnosis and establishes a specific diagnosis. The comparison between CS and CB was analysed by Chi-square test & kappa test. Results:Out of the 50 cases, 35(70%) were given a benign diagnosis, 10 (20%) were positive for malignancy, 2(4%) were suspicious and 3(6%) were inadequate for opinion on CS. In CB out of 50 cases, 29 of them confirmed/established a diagnosis and 21 cases were non diagnostic / non-contributory. CB gave an improved diagnosis in 2 out of 10 (20% ) malignant cases and 2 out of 35 (5.7%) benign cases. ( p value = 0.00054, Kappa value =0.34) Conclusions: CBs complemented CS, more so in malignant lesions by preserved architecture. Aspirates from multiple sites of the cystic lesions (with/without radiological assistance) pooled as one specimen yielded better material for CBs and ancillary techniques like histochemistry and IHC.

Purpose To systematically review (SR) and critically appraise studies that investigated the diagnostic capability of fine‐needle aspiration cytology (FNAC) in the assessment of intraosseous lesions of the jaws. Methods Six main electronic databases and three gray literature databases were searched aiming diagnostic studies, which were selected in a two‐phase process. Risk of bias (RoB) of included studies was assessed using the Revised Tool for the Quality Assessment of Diagnostic Accuracy Studies. Results A total of eight articles were included, of which six evaluated the FNAC smear technique exclusively, one assessed both smear and cell‐block techniques, while one investigated only the cell‐block technique. With regard to the FNAC smear technique, from 134 benign lesions investigated, concordance with the histopathological diagnosis was achieved in 64.17% of cases. In addition, considering the 32 malignant lesions assessed through this technique, concordance with histopathology was achieved in all cases. Regarding the cell‐block technique, only benign lesions (n = 40) were evaluated in the included studies, in which the concordance rate with histopathology was of 77.5%. The overall RoB judgment was “at risk of bias” for seven included studies mostly due to lack of information or reportedly absence of blinding of evaluator. Conclusions Within the limitations of this SR, FNAC presented modest to satisfactory diagnostic capability for the assessment of intraosseous jaw lesions, showing concordance rates higher than 64% for the smear and 77% for the cell‐block technique. Also, considerably high concordance rates were found for the assessment of malignant lesions regarding FNAC smear technique.
