Cecal Volvulus

Abstract: A twisted loop of the bowel and its mesentery on a fixed point is known as volvulus and it may arise more frequently in the sigmoid colon and cecum. Cecal volvulus as an uncommon cause of acute intestinal obstruction is axial twist of the cecum, ascending colon and terminal ileum around their mesenteric pedicles. Although there are many different etiologic and predisposing factors for cecal volvulus, exact etiology is most likely multifactorial in presence of mobile cecum. Its clinical presentation is highly v… Show more

“…4 Cecal volvulus is associated with an incomplete fixation of the cecum during embryogenesis. 4,5 Normally during embryogenesis, the primitive gut tube lengthens and twists around the superior mesenteric artery 90 degrees counterclockwise outside the abdominal cavity. The primitive gut tube continues to twist around itself 180 degrees as it returns into the abdominal cavity.…”

“…This results in the location of the appendix, cecum, ascending colon, and two-thirds of the transverse colon in the right lower portion of the abdominal cavity. 5 Transverse volvulus is a rare event, because the anatomy does not easily permit twisting as compared to the cecum due to its shorter length. TOTAL COLONIC VOLVULUS…”

“…However, laparoscopic cholecystectomy is associated with several possibly life-threatening complications, such as injury to the biliary tract, vessels, and the adjacent bowel 1 . Cecal volvulus is characterized by torsion of the cecum around its own mesentery and often results in obstruction, ischemia, necrosis, and perforation 2 . However, cecal volvulus is a rare complication of laparoscopic cholecystectomy.…”

“…In cases of axial volvulus, the caecum rotates around its long axis. In the caecal bascule subgroup, an upward folding of the caecum occurs, either anteriorly or posteriorly . Through either mechanism, the twisting or folding results in proximal large bowel obstruction with the risk of sequelae of ischaemia and perforation.…”

“…Caecal volvulus refers to the malrotation of the caecum and can be subdivided into two major subgroups: axial ileocolic volvulus which accounts for 90% of cases and caecal bascule which accounts for the remaining 10%. 1 In cases of axial volvulus, the caecum rotates around its long axis. In the caecal bascule subgroup, an upward folding of the caecum occurs, either anteriorly or posteriorly.…”
