Case summaryThis is a 45-year-old man with history of C3 quadriparesis secondary to trauma at the age of 3 years, who presented with slurred speech and increased difficulty using his left arm and leg for 3 months. He had received metronidazole 500 mg twice daily for 5 months for chronic clostridium difficile infection. Neurological exam was significant for dysarthria. Brain MRI showed symmetrical hyperintensities in the cerebellar dentate nuclei and splenium of the corpus callosum on DWI (Fig. 1a, b) and T2 FLAIR (Fig. 2a, b) without corresponding clear decreased intensity on ADC. These distinctive MRI findings have been described in metronidazole toxicity [1].Upon stopping metronidazole, the patient's dysarthria resolved within a day. Repeat brain MRI 4 months later showed resolution with minimal residual findings. Previous reports show that MRI changes are reversible.
