Case Report: A Severe and Multi-Site Nocardia farcinica Infection Rapidly and Precisely Identified by Metagenomic Next-Generation Sequencing

Abstract: Nocardia genus is an aerobic, gram-positive, and opportunistic pathogen, which mainly affects cell-mediated immunosuppressed patients. Early diagnosis and treatment greatly improve prognosis. However, the limitation of golden standard-bacterial culture exists. Here, we report a 61-year-old male with pneumonia, sepsis and intermuscular abscesses induced by Nocardia farcinica. Venous blood culture reported negative results. Former improper diagnosis and treatment did not improve his condition. With the assistant… Show more

“…The route of infection is usually via inhalation or direct inoculation ( 12 ). Commonly reported human infections with Nocardia are pulmonary nocardiosis, cutaneous nocardiosis, and brain abscesses ( 13 , 14 ), while there are also some reports about endophthalmitis caused by Nocardia ( 4 ). Nocardia endophthalmitis is considered of a relatively rare but potentially devastating ocular condition ( 15 ).…”

“…Isolation and culture are the principal methods for diagnosis of nocardiosis 21 . At present, isolation of Nocardia on common or selective media usually takes 2-7 days, which causes a delay in diagnosis 22,23 . Some Nocardia strains even need between 1-4 weeks to generate available results, because they require strict culture conditions and have a slow growth rate at 35°C in standard culture medium; the presence of normal ora or treatment with antimicrobial therapy would also affect Nocardia culture 11,13,14,24,25 .…”

“…5 Jiao et al has described use of Next-Generation Sequencing and its clinical use. 24 Relapses and multiple episodes have been known to occur in cases of nocardiosis, especially in patients with underlying immunosuppression. In a study on 132 patients of nocardiosis, 4 patients had at least 2 episodes at the same or different sites over a gap of 6-26 months with similar or different Nocardia isolates.…”
