Carnitine for fatigue in multiple sclerosis
Abstract: There is insufficient evidence that carnitine for the treatment of MS-related fatigue offers a therapeutic advantage over placebo or active comparators. Results of the ongoing trial are eagerly anticipated in order to provide clarity.

“…While this pattern partially replicates our baseline findings, it also suggests a broader link between carnitine-related metabolic disruption and the physical and emotional symptom domains in chronic pain. These results align with prior studies that have implicated carnitine metabolism in fatigue and impaired physical performance across chronic conditions (Liu et al, 2022; Tejani et al, 2012), which suggests that disruptions in energy metabolism may contribute to the broader symptom burden observed in chronic pain.…”

“…Therefore, in this study, we compare modafinil and L-carnitine to prove their effectiveness in the treatment of fatigue by measuring the MIFS before and after 2 months of treatment. [ 17 18 ] The study found that both treatments significantly reduced fatigue by reducing the MIFS, and this is consistent with some studies conducted on patients with MS who suffer from fatigue; the greatest impact was in patients who use L-carnitine. The serum carnitine levels were reported to be lower in MS patients who suffered from fatigue compared to those who did not.…”

“…Systematic reviews provide some evidence in favour of drug therapies and energy conservation treatment, although studies have yielded mixed results. [30][31][32] Results from a randomised controlled trial, and confirmed in a meta-analysis (albeit with a mixed disease type), support the potential benefits of exercise on fatigue. 34,84 Randomised controlled trials of vestibular rehabilitation and patient education programmes that incorporate a cognitive behavioural approach also provide some evidence of benefit, [85][86][87] as have some, 88 but not other, 89 multifaceted rehabilitation studies.…”
