Cardiac MR Imaging of Nonischemic Cardiomyopathies: Imaging Protocols and Spectra of Appearances

Abstract: Recent technologic advances in cardiac magnetic resonance (MR) imaging have resulted in images with high spatial and temporal resolution and excellent myocardial tissue characterization. Cardiac MR is a valuable imaging technique for detection and assessment of the morphology and functional characteristics of the nonischemic cardiomyopathy. It has gained acceptance as a standalone imaging modality that can provide further information beyond the capabilities of traditional modalities such as echocardiography an… Show more

“…Although many patients are asymptomatic with preserved LV systolic function, others develop heart failure, thromboembolic events, and malignant arrhythmias. Heart failure symptoms varying from mild to severe have been observed in approximately half of patients (7). Systolic dysfunction and arrhythmias may be a consequence of dysfunction at the microcirculation level and subepicardial hypoperfusion (4).…”

“…Furthermore, in patients with myocarditis, EMB provides not only diagnostic clues but also therapeutic and prognostic implications (16, 17). Despite all advantages of EMB-based diagnosis, EMB procedure itself is perceived to carry a high risk of major adverse events, such as cardiac tamponade and some raise a question about the role of EMB in the era of new cardiac imaging modalities such as cardiac magnetic resonance imaging (18, 19). However, histological confirmation is still needed in many patients in whom non-invasive evaluation is inconclusive.…”

“…In dilated cardiomyopathy, the LV is dilated and there is global systolic dysfunction. A nonischemic pattern of LGE may be seen, in a mid-myocardial or subepicardial distribution depending on the underlying aetiology [36]. A linear mid-myocardial pattern has been described in idiopathic dilated cardiomyopathy [36].…”
