Carcinoid in an umbilical nodule

Abstract: An umbilical nodule may be an early or late sign of metastasis from an internal location. To our knowledge, we are reporting the first case of carcinoid occuring as an umbilical mass. The occurrence in our patient of idiopathic hyperparathyroidism, diabetes mellitus, and carcinoid suggests the diagnosis of multiple endocrine adenomatosis, type I, which may result from a developmental defect in the amine and amine precursor uptake and decarboxylation cell system.

“…The site of the primary tumor was the bronchus in 13 (46.4%) cases 9–15,18,21,22,25 the gastrointestinal tract in 8 (28.5%) cases, 4–8,23,26 , the larynx in 2 (7.1%) cases, 20,24 and the testis in 1 (3.5%) case 19 . In the remaining four reported cases the primary site of the tumor was unknown 16,17,25 . In bronchial carcinoids, cutaneous metastases were found to be present at the time of diagnosis in five cases, whereas, in two cases, a metastatic nodule was the first clinical evidence indicative of the tumor 18,21,22 .…”

“…Cutaneous metastases from carcinoid tumors are only occasionally encountered in clinical practice. To the best of our knowledge, only 28 well‐documented cases have been reported so far in the literature 4–26 . Mean age of reported patients was 56 years (range: 35–80 years) with no difference in gender distribution.…”

“…In a search of the English literature, 17 cases of skin or subcutaneous metastasis from carcinoid tumors were found. In nine the primary site was the bronchus, (McBurney et al 1953, Kirkler et al 1958, Toomey & Felson 1960, Reingold & Escovitz 1960, Jatlow & Rice 1964, Hyman & Wells 1964, Rudner et al 1965, six arose from the gastrointestinal tract (Norman et al 1971), and in two additional patients, the site of the primary tumor could not be determined (Levine et al 1963, Brody et al 1978. In another patient the skin was believed to be the primary site of origin of the carcinoid tumor (Van Dijk & ten Seldam 1975).…”

“…Metastases to the skin are uncommon, with only 35 cases reported in the literature, most as individual case reports . In five of these cases, the cutaneous metastases were the first clinical manifestation of the primary tumor in otherwise asymptomatic patients . As visceral malignancies only rarely disseminate to the skin and the clinical presentation of such lesions is usually non‐specific, the finding of such a metastatic neuroendocrine tumor may present a diagnostic challenge to the pathologist, particularly if the patient has no known history of malignancy.…”
