Children with urinary tract infection present with vague clinical features and candiduria in such patients signifies pyelonephritis/cystitis, disseminated candidiasis, contamination or colonisation. This study was aimed to determine the prevalence of candiduria in paediatric patients, their speciation, associated risk factors and antifungal susceptibility of these isolates which will help the clinicians in the better management of candiduria. In our study, the isolation rate for candiduria was 24.3%, more common in males (63%) compared to females (37%) and infants were most commonly involved. Colony count of ≥10 5 cfu/ml seen in 53% of the isolates and pus cells were seen in 84% cases. Among candiduria cases, 87% received broad spectrum antibiotics for more than 7 days, while 57% were catheterized, and 19% of the patients were on fluconazole therapy. In the present study, non-albicans candida species emerged as the predominant pathogen and was found in 57% cases while Candida albicans was found in 43% cases of candiduria. Antifungal susceptibility testing revealed that all the isolates were sensitive to voriconazole and ketoconazole while 95% of the isolates were sensitive to fluconazole and 92% were sensitive to itraconazole. Resistance to fluconazole was found in 5% of isolates while 8% isolates were resistant to itraconazole. Identification of candida species along with antifungal susceptibility is important in patients with UTI and will help the clinician in selecting the appropriate antifungal agent for better management of such cases.

Candiduria refers to the presence of Candida species in urine, It is a common nosocomial infection afflicting the urinary tract. The aim of our study was to determine the prevalence of candiduria in hospitalized patients in Dschang District Hospital, and to evaluate the susceptibility patterns of Candida spp isolated from some commonly used antifungal drugs. Midstream urine samples were collected from 122 hospitalized patients at the Dschang District Hospital, then speciation of Candida was done based on colony colour on CHROMagar Candida medium. Antifungal susceptibility testing of the isolates was performed by the Broth dilution method. A total of 51 isolates of Candida species were recovered from 122 urine samples. Candida albicans was the most prevalent species 41.2% (21 isolates), followed by C. parapsilosis (27.5%), C. glabrata (17.6%), C. krusei (7.8%) and C. tropicalis (5.9%). Antifungal susceptibility test indicated that 100% and 96.1% of the Candida isolates were resistant to Amphotericin B and Nystatin respectively. Resistance varied depending on the species and the respective antifungal agents. We can conclude that candiduria is relatively common in hospitalized patients. The isolates were very resistant to polyenes (amphotericin B and nystatin) and most isolates were sensitive to azoles. Therefore, the species identification of Candida isolates along with their antifungal susceptibility pattern can help the clinicians in adequate treatment of the patients with candiduria.

Background: A broad variety of fungi, especially Candida species, are becoming increasingly common causes of urinary tract infections in hospital settings. Objectives: The current cross-sectional descriptive study aimed to determine the causative agents, risk factors and incidence rate of candiduria in critically ill patients, hospitalized in intensive care units (ICUs) of four Iranian hospitals. Patients and Methods: A total of 155 children and adult patients, admitted to ICUs of the four university educational hospitals, who demonstrated Candida-positive urine cultures, were examined. Samples were processed via mycological procedures by direct microscopy and culture. Results: Candiduria was confirmed in 50 (32.26 %) patients and Candida albicans was the most frequently isolated species representing 60.0% of isolates, followed by 14.0% C. glabrata, 12.0% C. parapsilosis, 10.0% C. krusei, and 4.0% C. tropicalis. Most patients were female (58%) with a mean age of 46.7 years old. Generally, 39.7% and 62% of adults and children showed candiduria, respectively. The commonest predisposing factors were antibiotic therapy (98.0%), urinary catheterization (92.0%), corticotherapy (84.0%), being female (42.6 %), use of feeding tube (56%), and extended hospitalization, respectively. Conclusions: The high frequency of candiduria in ICU patients can be decreased by shortening the duration of urinary catheterization, avoiding extra antibiotics and corticosteroids, as well as controlling the predisposing factors and underlying conditions.
