Calcineurin inhibitors in HLA-identical living related donor kidney transplantation

Abstract: This study confirmed that there are no benefits of CNIs for HLA-identical LRD recipients. Moreover, we did find evidence of potential harm. Thus, monotherapy or early discontinuation of CNI should be given consideration in these patients.

“…There was no difference in patient and graft survival between those who did receive CNIs, with a trend toward higher rates of chronic allograft nephropathy in CNI-exposed patients, leading to the conclusion that CNI maintenance was not warranted in this patient population. 25 Our study confirms these earlier findings by showing that CNIs can be withdrawn in white recipients of 2-haplotype matched living related kidneys, but extends these findings by further showing that this privileged group of patients do not require induction therapy. This is an important observation with implications for the immunosuppressive management and overall costs of care for these patients.…”

“…But, were permanent IS to continue significant morbidity and mortality would occur even in the modern era (16). That regulatory T cells might play a predominant role in HLA identity is not surprising, given the receptive environment in which they can act (17, 18).…”

“…Some studies reported superior outcomes when calcineurin inhibitors were added, 7,8 while others indicated it might not be needed. [9][10][11] The subjects in this group had lower rates of acute rejection (10%), although few were on induction therapy and none took calcineurin inhibitors due to service protocol restrictions.…”
