Breast sarcoma. A case report and review of literature

Abstract: HighlightsBreast sarcoma is a rare but aggressive entity.Core biopsy will be the procedure of choice for the diagnosis.Lymph node metastases are uncommon and surgery represents the only potentially curative therapy.Tumor size and, specially, an adequate resection margin are the most important prognostic factors and determinant of long-term survival. There is no consensus in the use of adjuvant therapy, it will depend mainly on the risk of tumor recurrence.

“…On one hand, primary sarcomas can take various histological types that often require IHC to differentiate [9], and secondary sarcomas often present as angiosarcoma, especially after radiation therapy of another tumor [10]. In our patient, it was important to do IHC to confirm the diagnosis as it is uncommon to be considered a different diagnosis in a previous carcinoma patient who never received radiotherapy.…”

“…In the case of suspicion of a breast AS, the best imaging test is MRI [9,30,31]. The final diagnosis of AS is made based on the results of pathological testing [32,33].…”

“…Moreover, undifferentiated pleomorphic sarcoma in the breast is an aggressive, fast-growing tumor with a high rate of local recurrence (44%) and distal metastases (42%) [ 1 ], but the therapeutic recommendations are difficult to establish with evolving techniques and limited patient numbers. The most important treatment element in maximizing disease-free survival and overall survival is complete surgical removal of the tumor [ 32 36 ], although a standard surgical practice is still not known, owing to the rarity of this tumor. In the absence of prospective and randomized trials, response rates to systemic therapies remain poor [ 37 ].…”
