Bone Healing in Adolescents with Bilateral L5 Spondylolysis with and without Preexisting Contralateral Terminal Spondylolysis

Abstract: To examine bone healing with conservative treatment in cases of adolescent bilateral L5 spondylolysis. Overview of Literature: We classified bilateral L5 spondylolysis, which is the most affected spinal level, by fracture stage and aimed to compare the bone healing rate according to the fracture stage and evaluate the presence of a preexisting contralateral terminal fracture at the diagnosis of fresh spondylolysis. Methods: We evaluated 48 patients (38 boys and 10 girls) with bilateral L5 spondylolysis diagnos… Show more

“…Many risk factors for bone healing have been reported in the conservative treatment of adolescent lumbar spondylolysis, including progressive, L5, bilateral, and contralateral terminal-stage lesions [ 3 , 5 , 10 , 11 ]. Many reports describe lesions that do not respond well to conservative treatment, such as progressive-stage lesions with a contralateral terminal-stage lesion [ 10 , 11 ]. In contrast, there have been few reports of lesions with good outcomes and lesions with consistent bone healing.…”

“…In this study, the bone healing rate of pre-lysis-stage lesions was 95%, with 5% of the lesions failing to heal. Bilateral lesions have been reported as a risk factor for conservative treatment in previous studies involving all stages of the disease [ 3 , 5 , 11 ]. Even in pre-lysis-stage lesions, the bone healing rate in bilateral lesions is lower than in unilateral lesions.…”
