Abstract: A 69-year-old gentleman with non-Hodgkin's lymphoma (stage I), with baseline fibrotic lung changes on CT, received six cycles of R-PMitCebo chemotherapy containing bleomycin. Three months later he presented to the Accident and Emergency Department with progressive dyspnoea, dry cough, pyrexia and generalised lethargy. Chest radiographs showed bilateral lower zone opacities. Clinically, all signs initially pointed to community-acquired penumonia, but he failed to respond to standard treatment for this. Repeat h… Show more

“…Bleomycin is used for the treatment of various types of malignancies, such as both Hodgkin and non-Hodgkin lymphoma and germ cell tumours [ 2 4 ]. However, bleomycin chemotherapy has been associated with various toxicities, such as skin rashes, mucositis, hypersensitivity reactions and bleomycin pulmonary toxicity [ 5 ].…”

“…Its incidence has been reported as >39.8% [ 6 ] and mortality ranges from 1% [ 7 ] to 2.8% [ 5 ] of the patients treated with bleomycin. BIP fibrosis has been described in 6–10% of cases receiving bleomycin [ 4 ].…”

“…There must be caution in extrapolating any results from animal studies to humans [15]. Some case reports have suggested that high-dose corticosteroids, including prednisolone at 1 mg/kg [4,9,16] and methylprednisolone up to 1 gm/m 2 for at least 5 days, with prolonged weaning may be successful in management of BIP [15], but only a few cases showed some benefit. Azathioprine has been used successfully as a corticosteroid sparing agent in a case of BIP, when preceded by high-dose methylprednisolone [17].…”

“…Bleomycin is recognised to cause an interstitial pneumonitis that can lead to fibrosis 1. The diagnosis of bleomycin induced pulmonary toxicity is often one of exclusion by sputum analysis, serology or antigen detection for micro-organisms 2. Patients with BIP are often treated for infection unsuccessfully before the diagnosis is confirmed.…”
