Biochemical Biomarkers of Mucosal Healing for Inflammatory Bowel Disease in Adults

Abstract: Mucosal healing (MH) is the key therapeutic target of inflammatory bowel disease (IBD). The evaluation of MH remains challenging, with endoscopy being the golden standard. We performed a comprehensive overview of the performance of fecal-, serum-, and urine-based biochemical markers in colonic IBD to find out whether we are ready to replace endoscopy with a non-invasive but equally accurate instrument. A Pubmed, Web of Knowledge, and Scopus search of original articles as potential MH markers in adults, publish… Show more

“…We decided not to include data on CRP or imaging markers (magnetic resonance imaging, ultrasound) in detecting MH. A previous comprehensive analysis regarding the role of CRP in this setting was recently published by Krzystek-Korpacka et al [ 11 ]. In this review of 30 studies, there was a large variation in the selected optimal cutoff values, ranging from 0.4 mg/L to 28 mg/L.…”

“…Neither imaging markers nor urine-based markers were included in this analysis. Studies investigating the role of C-reactive protein (CRP) in detecting MH were also not included, as a comprehensive analysis regarding this subject was recently published[ 11 ]. In this review, 30 eligible publications regarding CRP were included.…”

“…Regarding fecal biomarkers, there is still controversy about the optimal cut-off points determining remission. However, as shown on the table, the mean values for calprotectin and lactoferrin were above the suggested thresholds of remission [ 11 ].…”

“…Therapeutic strategy in UC has evolved into a "treat to target" approach. The most commonly accepted therapeutic targets became endoscopic remission (defined as Mayo Endoscopic Score; MES ≤ 1) and clinical remission, that is, patient-reported outcome (defined as resolution of rectal bleeding and diarrhea/altered bowel habit) [115,116]. Both current and novel treatment options are now evaluated based on their efficiency in the mucosal healing (MH) induction [117].…”
