These three new cases and the review of 43 cases reported in the literature call attention to the clinical features of carcinomas arising in the context of rhinophyma, raising further concerns about the possible association between these two entities. The need for histologic examination of all surgically removed tissue in patients with rhinophyma is highlighted. Several macroscopic changes, including ulceration, drainage, and a rapid growth pattern, should alarm the physician and should be considered as suspicious of a malignant degeneration. Unexpected clinical modifications of a preexisting long-lasting silent rhinophyma could indicate the possibility of hidden malignancy rather than a rhinophyma itself. Although evidence of an association between the two entities remains inconclusive, half of the malignancies reported in our review were incidental findings associated with rhinophyma. Thus, since rhinophyma should not be considered solely a cosmetic problem, we recommend that all specimens be reviewed by a pathologist and if malignancy is diagnosed, re-excision with clear margins should be achieved when necessary with periodic follow-up.

Abstract:Rhinophyma is characterized by progressive hyperplasia of nasal sebaceous glands, with fibrosis, follicular plugging and telangiectasias. The term "rhynophyma" derives from the Greek words rhino (meaning nose) and phyma (meaning growth). It is considered the final stage of acne rosacea. Though rare in occurrence, malignant tumors can grow over rhinophymatous skin, with basal cell carcinoma (BCC) being the most commonly reported tumor. Although a rare event, the development of BCC should always be sought in presence of rhinophyma, and also small changes of the skin lesion characteristic that it represent. We report four cases of BCC arising on rhinophyma observed between 1998 and 2012. We recommend the need for a histologic examination of the suspicious lesions or all surgically removed tissue in patients with rhinophyma.

Basal cell carcinoma (BCC) is the most common skin tumor of the white race. There are four types including nodular, pigmented, superficial and morpheaform. The most common type is nodular type. Rhinophyma, represents the end stage of rosacea, and is caused by a progressive hypertrophy of sebaceous glandular tissue, connective tissue, and blood vessels. Besides, fibrosis, follicular plugging and telangiectasia are seen. Although rhinophyma is not a premalignant lesion, this relationship is considered by reports of malignancies on the background of rhinophyma. Various malignancies, including BCC, SCC, angiosarcoma and sebaceous carcinoma, have been reported to coexist or to imitate rhinophyma. We herein, report a 83 year patient presented with rhinophymatous lesion which was BCC originally, and emphasized the importance of careful examination of the skin and diagnostic biopsy of uncertain areas.
