Abstract: The aim of this study was to evaluate the bacterial translocation in liver transplantation (LT), comparing the conventional and the piggyback methods. A total of 32 patients were randomized into the 2 groups. Samples of blood were collected from the radial artery, portal vein (PV) and hepatic vein (HV), in up to 120 minutes postreperfusion. The samples were sent for endotoxin level, as well as samples up to 2 minutes post-perfusion were sent to culture. Hepatic artery and PV blood flows were measured at postre… Show more

“…It is conceivable that incisions to the skin for any kind of surgery, including cardiac transplantation, may allow some cutaneous bacteria to enter the bloodstream and signal at distant sites and/or that intestinal bacterial translocation during open abdominal surgery, as occurs in our model of heterotopic cardiac transplantation, may contribute to the enhanced alloimmunity observed in colonized compared with GF animals. Indeed, portal endotoxin has been detected after abdominal surgery that did not involve intestinal sectioning (31).…”

“…The most important finding in the study by Abdala et al 1 in our view is the lack of hepatic clearance of endotoxin shortly after liver transplantation. We however question the conclusion of the authors that hepatic endotoxin clearance might be diminished solely because of an alteration of the reticuloendothelial system.…”

“…We read with great interest the study by Abdala et al, 1 which examines whether there is significant bacterial translocation during liver transplantation. This is of particular interest because we have recently shown that endotoxin can impair immune function in the context of liver disease, which may have significant ramifications for recovery.…”

“…On the contrary, female patients and some liver disease may provide better prognosis to patients with HCC, but this is possibly more because of higher compliance with surveillance than to real biological differences 28,29 . Moreover, some of others donor-, In randomized Brazilian reports, the piggy-back approach was related to higher rate of pulmonary infiltrates 38 , but not to bacterial translocation 39 or stimuli for the production of inflammatory molecules 40 , when compared to the conventional method. However, until publication of this study, there were no trials comparing piggy-back with conventional method without veno-venous bypass 37 .…”
