According to the 36-Item Short Form Health Survey questionnaire developers, a global measure of health-related quality of life such as the “SF-36 Total/Global/Overall Score” cannot be generated from the questionnaire. However, studies keep on reporting such measure. This study aimed to evaluate the frequency and to describe some characteristics of articles reporting the SF-36 Total/Global/Overall Score in the scientific literature. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses method was adapted to a scoping review. We performed searches in PubMed, Web of Science, SCOPUS, BVS, and Cochrane Library databases for articles using such scores. We found 172 articles published between 1997 and 2015; 110 (64.0%) of them were published from 2010 onwards; 30.0% appeared in journals with Impact Factor 3.00 or greater. Overall, 129 (75.0%) out of the 172 studies did not specify the method for calculating the “SF-36 Total Score”; 13 studies did not specify their methods but referred to the SF-36 developers’ studies or others; and 30 articles used different strategies for calculating such score, the most frequent being arithmetic averaging of the eight SF-36 domains scores. We concluded that the “SF-36 Total/Global/Overall Score” has been increasingly reported in the scientific literature. Researchers should be aware of this procedure and of its possible impacts upon human health.

Neighborhood Socioeconomic Status Predicts Health After Hospitalization for Acute Coronary Syndromes

Objective To explore the influence of contextual factors on health-related quality of life (HRQoL), which is sometimes used as an indicator of quality of care, we examined the association of neighborhood socioeconomic status (NSES) and trajectories of HRQoL after hospitalization for acute coronary syndromes (ACS). Methods We studied 1,481patients hospitalized with ACS in Massachusetts and Georgia querying HRQoL via the mental and physical components of the SF-36 (MCS and PCS) and the physical limitations and angina-related HRQoL subscales of the Seattle Angina Questionnaire (SAQ) during hospitalization and at 1-, 3-, and 6-months post-discharge. We categorized participants by tertiles of the Neighborhood Deprivation Index (a residence-census tract-based measure) to examine the association of NSES with trajectories of HRQoL after adjusting for individual SES and clinical characteristics. Results Participants had mean age 61.3 (SD: 11.4) years; 33% were female; 76%, non-Hispanic white; 11.2% had household income below the federal poverty level. During 6 months post-discharge, living in lower NSES neighborhoods was associated with lower mean PCS scores (1.5 points for intermediate NSES; 1.8 for low) and SAQ scores (2.4 and 4.2 points) versus living in high NSES neighborhoods. Neighborhood SES was more consequential for patients with lower individual SES. Individuals living below the federal poverty level had lower average MCS and SAQ physical scores (3.7 and 7.7 points, respectively) than those above. Conclusions Neighborhood deprivation was associated with worse health status. Using HRQoL to assess quality of care without accounting for individual and neighborhood SES may unfairly penalize safety net hospitals.

Introdução: As doenças cardiovasculares somam 1/3 de todas as mortes na América Latina e no Brasil, representando a 2ª causa de morte no país. Objetivos: Mensurar e acompanhar a qualidade de vida relacionada a saúde (QVRS) durante 180 dias e correlacionar os resultados com ansiedade, depressão em pacientes submetidos a cirurgia de revascularização do miocárdio (CRM). Métodos: 63 pacientes foram submetidos a CRM eletiva, no pré-operatório foram coletados os dados sociodemográficos e clínicos depois os pacientes preencheram questionários: MacNew QLMI para avaliação de QVRS, IDATE Traço-Estado para avaliação de ansiedade e Beck Depression Inventory (BDI) para avaliação de depressão. Após a alta hospitalar, em seus domicílios, os pacientes responderam novamente os questionários e foram acompanhados por 6 meses. Resultados: Foi observada melhora significativa da QVRS após 60 dias, que se manteve estável entre 120-180 dias após CRM. Houve uma correlação negativa significante entre QVRS e depressão e QVRS e ansiedade. Quanto aos domínios do QLMI, observou-se melhora significante no físico e social após 120 e 180 dias da cirurgia. Não foi observada diferença significativa nos escores de depressão e ansiedade em relação ao tempo de CRM. Porém, observou-se redução significativa nos níveis de depressão 180 dias após CRM. Conclusão: Pacientes submetidos a CRM apresentam uma melhora espontânea significativa em sua QVRS após 60 dias e que se estabiliza até 180 dias. Fatores como ansiedade e depressão podem afetar o curso de sua completa recuperação emocional, física e social.Palavras-chave: ansiedade, depressão, qualidade de vida, cirurgia de revascularização do miocárdio.
