Abstract: Background: Despite recent advances in understanding its pathophysiology and development of novel therapies, asthma remains a serious public health issue worldwide. Combination therapy with inhaled corticosteroids and long-acting β 2-adrenoceptor agonists results in disease control for many patients, but those who exhibit severe asthma are often unresponsive to conventional treatment, experiencing worse quality of life, frequent exacerbations, and increasing healthcare costs. Bone marrow-derived mononuclear ce… Show more

“…Therefore, there is an increasing demand to use new methods for treating asthma based on the modulation of Th2 cells and their responses (Urbanek et al., 2016). Nowadays, clinicians and researchers have come to the conclusion that stem cell therapy is a promising therapeutic modality to overcome lung disorders (Aguiar et al., 2020; Hosseinirad et al., 2018; Lee, Choi, Oh, & Lee, 2018). However, a suitable cell type has not been identified to restore multiple pathologies after lung disorders (Hosseinirad et al., 2018; Ramachandran et al., 2015).…”

“…The MSCs’ immunomodulatory properties affect proliferation, activation, and function of various immune cells ( Harrell et al, 2019 ) and may thus alter the innate and adoptive immune responses ( Li et al, 2016 ). The underlying cellular and molecular mechanisms of the long-term effects of MSC-mediation are yet not fully clarified, but regenerative and immunomodulatory effects have been observed in various diseases and tissue types ( Aguiar et al, 2020 ; Qin and Zhao, 2020 ; Sharma et al, 2020 ; Tao and Chen, 2020 ). Although studies describe MSCs as short-lived ( Eggenhofer et al, 2012 ), unable to cross the lung capillary network after intravenous infusion ( Fischer et al, 2009 ), there have been preclinical reports that survival of exogenous MSCs could be detected on site up to 4 months after their direct transplantation; however, few of these cells developed the tissue phenotype of the resident cells ( Muñoz et al, 2018 ).…”

“…However, there are still some issues that must be addressed before translating this therapy to humans. To the best of our knowledge, there is only one study (Aguiar et al, 2020) that demonstrated the potential efficacy of MSCs in patients with asthma and one ongoing clinical trial (NCT03137199). However, these studies (Aguiar et al, 2020) have limitations, such as a small sample size with no placebo-control and inflammatory biomarkers.…”

However, these studies (Aguiar et al, 2020) have limitations, such as a small sample size with no placebo-control and inflammatory biomarkers. Procedures for cell culture need to be appropriately standardized to produce a satisfactory number of cells and should be proliferative and maintain their regeneration properties.…”
