Introduction: The inappropriate use of medical technology is a major factor in increased health care expenses and inappropriate use of blood is costly. There is a need for continuous audit of the use of products as therapy. Audits are useful tools in the education of those ordering blood components, potentially resulting in the reduction of inappropriate use of blood components.

Aim: A retrospective clinical audit of blood transfusion requests in patients at a tertiary care hospital.

Material and Methods: This retrospective clinical audit conducted at NKP salve institute of medical sciences & research center & lata mangeshkar hospital Nagpur for a period of 3 months from April 2013 to June 2013. A retrospective analysis of blood and its component requisitions in all patients from different clinical departments was reviewed regarding diagnosis, indication for transfusion, number of units requested and the speciality prescribing it. Reports of silent investigations like hemoglobin, platelet count, coagulogram was also recorded.

Results: Out of 869 requisitions, 301 were for male patients and 568 were for female patients. Total number of blood components distribution request was 1188. 632 were for whole blood, 364 were for packed red cells, 182 were for FFP and 10 for platelet concentrate. Pre transfusion Hemoglobin was >8 gm/dl in 273 requests. Pre transfusion hemoglobin values were not available for 192. The urgency for transfusion was mentioned on 728, indication was mentioned on 804 and diagnosis was mentioned on 755 requisition forms. Multiple blood components requests were 97. The maximum request was from Department of Obstetrics and Gynaecology.

Conclusion: Clinical audits helps to reduce inappropriate use of blood and blood components, avoids blood transfusion reaction and avail the deprived patients.

Background: Audit is one mechanism for evaluating the effectiveness of the total quality system. Transfusion medicine or blood bank audit embraces all procedures from blood procurement to the long term consequences of transfusion. Aims and Objectives:To understand the problems arising in day to day working of blood bank and to ensure the appropriate indication for issuing blood and blood components to different departments. Materials and methods:Present audit of blood bank is retrospective study done for a year in tertiary care center in central region of India. Data collection of donors and whole blood issue is done for one year from 1st January 2016 to 31st December 2016. Component separation had started from August 2016, so audit of component separation and its usage has been done for one year from 1stAugust 2016 to 31stJuly 2017.Results: In year 2016, total blood units collected were 6131 and issued were 5897. Majority of blood issue was to surgical wards 3790 compared to non-surgical wards 2107. Out of 6109, 203 were positive for transfusion transmitted infections (TTI). 20 transfusion reactions were reported in 2016. From 1st August 2016 to 31st July 2017, total 1060 Packed Red Cells (PRC) were prepared and 1009 were utilized, 1120 units of FFP were prepared, out of which 778 were utilized and 817 platelet concentrates were prepared but only 205 is utilized. Conclusion:Results obtained from these types of retrospective studies will provide opportunity for finding strategies in improving the blood bank services.

IntroductionBlood bank service facility of blood collection, storage and dispatching began from 19 June 2016 in the new blood bank and transfusion service unit, at Birat Medical College & Teaching Hospital, Tankisinuwari, Morang, Nepal, with the support of Nepal Red Cross Society. As the supply of blood is finite, the inappropriate use of blood transfusion units and its component in the world exist, hence there is a need to monitor and regulate these services.ObjectivesThe objective of this study is to know and identify different blood groups and its utilization pattern to meet the demand for future preparedness and to identify the total blood unit, which are being, replaced back.MethodologyThis retrospective study was conducted at BMCTH, Tankisinuwari, Morang for component dispatches from 19th June, 2016 to 19th February, 2017. Requisitions for whole blood requirement for patients from various departments were reviewed regarding the department request, the whole blood requested and whether being replaced, blood group and socio-demographic profile.ResultsOut of 193 transfusion units dispatched, 28 patients were male and 165 patients were females respectively B positive blood groups were most common, followed by A positive, O positive, AB positive, both AB and O negate respectively. The largest numbers of requisitions were from Gynecology and Obstetrics ward in the hospital, while 22 requisites were from other health centers. Majority of the requisitions were for 51- 60 years age group from the local district Morang. The total number of blood units replaced were 38, out of which 5 for males and 33 for females respectively.ConclusionB positive was the predominant requested blood group, with Gynecology and Obstetrics ward demanding the most requests, among 51-60 years age group from Morang district.
