Association of red blood cell and platelet transfusions with persistent inflammation, immunosuppression, and catabolism syndrome in critically ill patients

Abstract: The objective of this single-center retrospective cohort study was to investigate the relationship between blood transfusion and persistent inflammation, immunosuppression, and catabolism syndrome (PIICS). The study was conducted at the Critical Care Center at Toho University Omori Medical Center, Japan. We included 391 patients in the PIICS group (hospitalization for > 15 days, C-reactive protein > 3.0 mg/dL or albumin < 3.0 mg/dL or lymph < 800/μL on day 14) and 762 patients in the non-PIICS grou… Show more

“…Blood transfusions has previously been demonstrated to induce a pro-in ammatory response independent of other treatments (e.g. surgery) (20), and studies have furthermore also linked perioperative transfusions to increased risk of cancer recurrence for pancreatic surgery patients (21) as well as other solid tumors (22,23,24). As such, our ndings are in line with these studies.…”

“… 22 This finding might be related to the fact that allogeneic blood transfusion induces inflammation, immunosuppression, and predisposes patients to postoperative infection. 32 , 33 A recent retrospective study found that blood cell transfusion has an OR of 2.02 for the development of postoperative complications in patients undergoing MIE. 34 In the present study, the PPCs group also had higher incidences of non-pulmonary complications than the non-PPCs group.…”

“…However, the great body of literature mentioned the harmful effect of transfusion immunomodulation on the patient’s immune function. They suggest patients with cancer may have an impaired immune system 5 and ‌blood transfusions lead to the deterioration of the natural process of the immune system in cancer cells elimination. In Ref.…”

“…Moreover, the relationship between blood transfusion and persistent inflammation, immunosuppression, and catabolism syndrome (PIICS) has been investigated in Ref. 5 , and based on that physicians should consider the risk of PIICS when performing blood transfusions in intensive care unit (ICU) patients.…”
