Breast cancer imaging phenotype is diverse and may relate to molecular alterations driving cancer behavior. We systematically reviewed and meta-analyzed relations between breast cancer imaging features and human epidermal growth factor receptor type 2 (HER2) overexpression as a marker of breast cancer aggressiveness. MEDLINE and EMBASE were searched for mammography, breast ultrasound, magnetic resonance imaging (MRI), and/or [ 18 F]fluorodeoxyglucose positron emission tomography studies through February 2013. Of 68 imaging features that could be pooled (85 articles, 23,255 cancers; random-effects meta-analysis), 11 significantly related to HER2 overexpression. Results based on five or more studies and robustness in subgroup analyses were as follows: the presence of microcalcifications on mammography [pooled odds ratio (pOR), 3.14; 95% confidence interval (CI), 2.46-4.00] or ultrasound (mass-associated pOR, 2.95; 95% CI, 2.34-3.71), branching or fine linear microcalcifications (pOR, 2.11; 95% CI, 1.07-4.14) or extremely dense breasts on mammography (pOR, 1.37; 95% CI, 1.07-1.76), and washout (pOR, 1.57; 95% CI, 1.11-2.21) or fast initial kinetics (pOR, 2.60; 95% CI, 1.43-4.73) on MRI all increased the chance of HER2 overexpression. Maximum [ 18 F]fluorodeoxyglucose standardized uptake value (SUV max ) was higher upon HER2 overexpression (pooled mean difference, þ0.76; 95% CI, 0.10-1.42). These results show that several imaging features relate to HER2 overexpression, lending credibility to the hypothesis that imaging phenotype reflects cancer behavior. The care of patients with breast cancer has become increasingly complex with advancements in diagnostic modalities, surgical approaches, and adjuvant treatments. A multidisciplinary approach to breast cancer care is essential to the successful integration of available therapies. This article addresses the key components of multidisciplinary breast cancer care in Netherland Cancer Institute, with a special emphasis on new and emerging approaches in the fields of diagnosis and treatment. 103 Meme kanseri tedavi yaklaşımlarını ve klinik işleyişlerini gözlemlemek amacıyla Netherland Cancer Institute/Antoni van Leeuwenhoek Hospital (NKI/AVL)'de kısa bir süre bulunma fırsatı elde ettim. Çok sayıda meslektaşımızın yakından bildiği üzere Avrupa'da devam eden çok sayıda çalışmaya aktif katılım gösteren bu merkezin klinik uygulamalarını yakından izledim. Meme kanserinde multidisipliner tedavi yaklaşımının önemine dikkat çe-ken bu merkezin tanı ve tedavi yaklaşımları ile güncel çalışmalarının özetini bulacağınız makalenin yararlı olmasını dilerim. Neoadjuvan tedavi yaklaşımlarıNeoadjuvan tedavi indikasyonları lokal ileri evre meme kanseri (LİMK) ile sınırlı değil. Tarama programları nedeniyle LİMK hasta oranlarının oldukça düşük olması dikkat çekici (1-5). Neoadjuvan tedavi çalışmalarının meme kanseri tedavi geleceğinde ve özgün çalışma üretiminde önemine inanmalarından dolayı hemen her aksilla pozitif veya T2 hastada tedaviye neoadjuvan kemoterapi (KT) ile başlanıyor (6-9).Enstitüde çeşitli ilaç kombinasyonlarının kullanıldığı birçok neoadjuvan çalışması devam ediyor. Neoadjuvan KT başlanma kararı verilen her hastada hem tümör yatağına hem de aksiler ultrasonografi (US) ile saptanan ve korteks kalınlığı en fazla olan nod içerisine yarı-lanma ömrü 60-90 gün olan I-125 ile işaretli seed yerleştiriliyor (10-12). Klinikte tanı amaçlı ince iğne aspirasyon biyopsisi (İİAB) tercih edilse de neoadjuvan KT adayı hastalarda US altında trucut biyopsi ve I-125 seed yerleştirilmesi genellikle eş zamanlı olarak yapılıyor. Aksillada şüpheli lenfadenopati varlığında İİAB yapılıyor, yine aksilla pozitif hastalarda saptanan korteksi en kalın ve patolojik olduğu düşünülen lenf noduna seed yerleştiriliyor. Aksillası İİAB ile negatif olup neoadjuvan tedavi adayı olan hastalarda neoadjuvan tedavi öncesinde sentinel lenf nodu biopsisi (SLNB) yapılıyor. SLNB sonucu pozitif olan hastalarda, neoadjuvan tedavi sonrasında her durumda aksiller diseksiyon uygulanıyor. Neoadjuvan tedavi öncesi SLNB ile aksillası negatif hastalarda KT sonrasında aksiller girişim yapılmıyor (13). Neoadjuvan KT öncesinde İİAB ile aksiller metastaz saptanan ve seed yerleştirilen hastalara ise KT sonunda cerrahi sırasında, önce aksiler küçük bir kesi ile seed in bulunduğu nod çıkarılıyor, arkasından diseksiyon yapılıyor (10). Özetle, aksillası neoadjuvan KT öncesinde İİAB veya SLNB ile pozitif olduğu saptanan her hastaya neoadjuvan KT sonunda aksiler diseksiyon (AD) yapılıyor. AbstractPrimary systemic therapy (PST) is a standard treatment for patients with locally advanced breast cancer. We report one of our patients to demonstrate the optimal use of FDG-PET-CT in the routine clinical workup during PST, especially when clinicians face contradictory clinical and pathological findings, and to show the advantages of this imaging modality in the decision-making process about the initial treatment choice. By reviewing the literature we would also like to confirm that FDG-PET-CT is highly sensitive in the measurement of the early therapeutic response and the prediction of the complete pathological remission, as early as after the first cycle of chemotherapy is administered.
