Association between COVID-19 vaccination and bullous pemphigoid – a case series and literature review

Abstract: Bullous pemphigoid is an acquired autoimmune blistering dermatosis that is commonly associated with high morbidity and mortality. New-onset bullous pemphigoid following vaccinations has rarely been reported in the literature. We report two cases of new-onset bullous pemphigoid after COVID-19 vaccination followed by a brief literature review. Twenty-six cases were included in our analysis. Our cases support that new-onset bullous pemphigoid may develop following COVID-19 vaccinations. Entering a period of clini… Show more

“…5 Additionally, reactions between SARS-coV-2 spike protein antibodies and tissue protein transglutaminases have been implicated, possibly leading to BP development. 6 Similar to the study by Arduino et al, we find higher incidences of pemphigus in females, with pemphigus vulgaris being the most common subtype. 1 We observe similar overall trends in BP patients.…”

“…It has been proposed that cross-reactivity between these particles leads to the development of autoimmune diseases such as bullous pemphigoid. 29 In our study, mRNA Covid-19 vaccine (Pfizer) was linked to BP. Pfizer vaccine is more often associated with vaccine-induced bullous pemphigoid than other mRNA-or vector-based vaccines.…”

“…In the flare group, 92% of cases were classified as 'possible', and 8% as 'probable'. Notably, all cases deemed 'probable' in causality had experienced a disease flare following both doses of COVID-19 vaccines, contributing to the overall score for these cases [6,13,25,37,38,[40][41][42]49,50,53,54,68,71,75,86,91].…”

“…In the new onset group, BP patients with limited involvement were treated with topical corticosteroids, while those with more extensive involvements received a variety of systemic immunomodulators, including corticosteroids, doxycycline, nicotinamide, methotrexate, azathioprine, cyclosporine, mycophenolate mofetil, cyclophosphamide, dapsone, colchicine, or hydroxychloroquine [5,6,14,15,28,31,32,35,38,39,[42][43][44][45]49,51,53,56,[70][71][72][73]75,77,82,86,87,92,93]. DPP-4 inhibitors were suspended in patients using these medications [34,44,45,48].…”
