Assessment in Severe Dementia: The Guy's Advanced Dementia Schedule

Abstract: Previous findings suggest that severely demented subjects are often capable of responding in some ways to stimulation. This study describes the development of a schedule which can discriminate between subjects on the basis of their responses to a range of objects of varying familiarity. The schedule includes extensive prompting to elicit a range of responses from simple taking of objects through to naming and using. The measure was found to be reliable and valid against the Clifton Assessment Procedures for th… Show more

“…Dementia researchers should reach international consensus about the terminology and operational definition of late dementia, to consolidate and compare studies. Designs should be developed to follow dementia patients until late dementia, and assessment scales should be constructed that will measure slight changes in this phase of the disease, such as changes in emotional expression or responding to stimulation 18–21 …”

“…• Cognitive functioning: Guy's Advanced Dementia Schedule [32]; a structured assessment of cognitive ability that involves measuring responses to familiar objects. [In the initial funding application we proposed to use the Severe Impairment Battery [33], but as initial piloting indicated floor effects on this measure we replaced it with the GADS.…”

“…COPD effects on cognition are still poorly understood; indeed, this comorbidity has not been investigated as extensively as other pathologies 10–13. Cognitive function impairment was found to be associated with severe pulmonary dysfunction long ago,1419 although its prevalence varied from study to study2023 and was dependent on the specific diagnostic criteria adopted, the methods used for assessing the impairment, and the number of subjects investigated 12,24,25. Several studies investigating cognition in subjects with chronic airflow limitation had methodological limitations, including lack of clinical assessment of airflow impairment;26 a specific focus on cognitive impairment in elderly subjects with severe respiratory conditions; a lack of interest in subjects without a defined respiratory disease;20,2729 a lack of extensive comparison of cognitive impairment in COPD subjects versus subjects with other chronic airway disorders and/or healthy subjects;23,29 a lack of multiple psychometric tools;30 and small sample sizes 7,21,31,32…”
