Appendiceal Mucocele of Mucinous Cystadenocarcinoma with a Cutaneous Fistula

Abstract: We report a novel case of cystadenocarcinoma forming an appendiceal mucocele with development of a skin fistula. The patient was a 75-year-old Japanese woman who originally presented with a skin ulcer on the right flank (inferior to the ribs and superior to the iliac bone) with mucus discharge. The serum concentration of carcinoembryonal antigen was elevated (57.4 ng/ml). Ultrasonography and computed tomography demonstrated a cystic mass with septations in the right iliac fossa. Fistulography from the skin ulc… Show more

“…CT can determine the relation between lesion and the neighbouring organs, and help confirm the diagnosis [15,16,19,20] . CT reveals a cystic mass with enhancing wall nodularly in the expected area of the appendix, especially in older patients, in whom AM should be considered [9,15,16,21] . AM could be visualized by CT in three of our patients.…”

“…Colonoscopy is also important for the diagnosis of synchronous and metachronous colon tumor. Elevated CEA levels have been described in neoplastic mucoceles ( 24 ) The spontaneous and surgery induced complications of appendiceal mucocele include intestinal obstruction, intussusceptions (21), intestinal bleeding (13,25), fistula formation (15) and volvulus ( 26 ). The worst complication is pseudomyxoma peritonei.…”

“…Many of unusual presentations reported for primary appendicular carcinoma are the results of fistula formation into the adjacent viscera such as the urinary bladder, bowel or vagina as well as extraperitoneally into retroperitoneal tissues or directly to the skin surface. Extraperitoneal spread is associated with relatively good prognosis by preventing the development of peritoneal carcinomatosis [8].…”

“…Chemotherapy and radiotherapy have also shown to be helpful [10,11]. Similarly, tumours with extraperitoneal extension requires excision of the involved tissue and fistulous truck besides right hemicolectomy [5,8].…”
