As observed in a recent RCT by St Peter et al and proposed by a Cochrane review, a noninferiority margin of 20% was considered a reasonable threshold to balance the lower efficacy of NOM with its potential benefits. In our analysis, NOM had a 36.6% failure rate in 1 year compared with 7.0% in the operative management group, indicating that appendectomy outperformed antibiotics and surpassed the noninferiority margin of prior RCTs .…”
