The aim of this study was to evaluate whether the number of women recruited for studies to establish the efficacy of medications approved for treatment of alcohol dependence (AD) and of alcohol withdrawal syndrome (AWS) is sufficient to reveal possible gender differences in the response to these medications and in suggesting the use of different doses in female patients. Our results show that the rates of women recruited for studies evaluating the efficacy of disulfiram (1%), benzodiazepines (3%), and anticonvulsants (13%) were too low to establish possible gender differences. The rates of women recruited for studies evaluating the efficacy of acamprosate (22%), naltrexone (23%), and nalmefene (30%) were higher and allowed evaluation of data obtained for female patients. Women receive medications for treatment of AD and/or AWS for which efficacy has been demonstrated in studies in which men were more largely represented.

GrundlagenHauptmerkmale einer "Alkoholabhän-gigkeit" sind das ausgeprägte Verlangen der Betroffenen nach der Substanz Alkohol ("Craving"), die verminderte Kontrollfähigkeit in Bezug auf den Beginn, die Menge und Dauer des Alkoholkonsums, die Toleranzentwicklung, die Sensitivierung und die psychovegetative Entzugssymptomatik. Im ICD-10-System der WHO wird die Diagnose anhand von sechs Kriterien beschrieben. Eine Alkoholabhängigkeit kann erst dann diagnostiziert werden, wenn innerhalb des letzten Jahres drei oder mehr dieser Kriterien erfüllt waren [14]. Die Diagnose des schädlichen Konsums kann bereits bei einer wiederholt alkoholbedingten Problematik im sozialen, zwischenmenschlichen, beruflichen oder rechtlichen Lebensbereich einer Person gestellt werden.Beim "Alkoholmissbrauch" stehen also die schädlichen Konsequenzen des Alkoholgebrauchs im Vordergrund, etwa wenn durch die Wirkung des Alkohols gefähr-liche Situationen entstehen (z. B. Trunkenheit beim Führen eines Kraftfahrzeuges). Hauptmerkmal hierbei ist ein schädliches Alkoholkonsumverhalten, das sich in wiederholten und deutlich nachteiligen Konsequenzen infolge des wiederholten Alkoholgebrauchs manifestiert [14]. Etwa 50 % der Personen mit Alkoholmissbrauch betreiben diesen auch zwei bis fünf Jahre später noch, aber lediglich etwa 10 % von ihnen, auch der Jugendlichen, entwickeln eine Alkoholabhängigkeit [21].Die Lebenszeitprävalenz, an einer Alkoholabhängigkeit zu erkranken, beträgt in den meisten westlichen Ländern 10-15 % für Männer und 5-8 % für Frauen. Insgesamt sind die Prävalenzen in den USA, Kanada, Deutschland, Australien und England höher als in mediterranen Län-dern wie Italien, Griechenland und Israel Ziel einer Akutbehandlung bei Alkoholabhängigkeit ist die "unmittelbare Remission", das heißt Nachlassen kör-perlicher und psychischer Krankheitssymptome, ohne dass jedoch zwingend eine Genesung erreicht wird. Neben der Beendigung oder Verminderung des exzessiven Alkoholkonsums wird eine weitgehende Beseitigung der damit verbundenen Entzugserscheinungen angestrebt. Spezielle Screeningverfahren können erste Hinweise liefern, ob eine Alkoholerkrankung vorliegt. Die Therapie der Alkoholabhängigkeit umfasst neben dem Entzug ein komplexes Therapiekonzept, das medikamentöse wie psychotherapeutische Elemente integriert und stationär oder ambulant erfolgen kann.Der Zwang Alkohol zu konsumieren ist ein Kriterium für die Abhängigkeit.
