Background Bacille Calmette-Guérin (BCG) is routinely given at birth in tuberculosis-endemic settings due to its protective effect against disseminated tuberculosis in infants. BCG is however contraindicated in HIV-infected infants. We investigated whether delaying BCG vaccination to 14 weeks of age affected vaccine-induced antibody responses to Haemophilus influenza type b (Hib)-conjugate, pertussis, tetanus and Hepatitis B (HBV) vaccines, in HIV-exposed uninfected (HEU) andunexposed uninfected (HUU) infants. Methods Infants were randomized to receive BCG at birth or at 14 weeks of age. Blood was taken at 14, 24, and 52 weeks of age and analyzed for Hib, pertussis, tetanus and HBV specific antibodies. Results BCG was given either at birth (106 infants, 51 HEU) or at 14 weeks of age (74 infants, 50 HEU). The timing of BCG vaccination did not influence the antibody response to any antigen studied. However, in a non-randomised comparison, HEU infants had higher Hib antibody concentrations at weeks 14 and 24 (p=0.001 and <0.001 respectively) and pertussis at week 24 (p=0.003). Conversely, HEU infants had lower antibody concentrations to HBV at 14 and 52 weeks (p=0.032 and p=0.031) with no differences in tetanus titres.

Diagnosis of acute human immunodeficiency virus (HIV) infection, a key driver of the HIV epidemic, remains a public health challenge. The PlasmAcute technology offers an opportunity to detect early anti-HIV antibody responses. B lymphocytes (B cells) were isolated from the blood of seronegative miners in South Africa by using the PlasmAcute method. B-cell lysates and paired sera were tested for anti-HIV-1 antibodies by two different enzyme-linked immunosorbent assays; immunoreactivity was confirmed by Western blotting. All volunteers were tested for HIV type 1 (HIV-1) viral load, p24 antigen, and CD4 count. Sera from HIV-seronegative men who had positive viral loads and were positive for p24 antigen were retested for anti-HIV antibodies after immune complex dissociation. Anti-HIV antibodies were detected in lysates from 16/259 subjects without immunoreactivity in paired sera. Four subjects, one of whom had a positive viral load initially, subsequently seroconverted. Six subjects showed transient anti-HIV-1 antibodies in the lysates and tested negative for all markers at the follow-up. Five subjects without follow-up data initially had lysate-positive/ serum-negative samples, and these cases were classified as inconclusive. One subject had lysate antibodies and a detectable viral load but was seronegative at follow-up. In conclusion, lysate-derived anti-HIV-1 B-cell antibodies can be detected prior to seroconversion and earlier than or contemporary with HIV-1 RNA detection.

Respuesta a la vacuna contra la hepatitis B en pacientes en hemodiálisis crónicaResponse to hepatitis B vaccine in patients with chronic hemodialysis ResumenIntroducción: la hepatitis B (HB) sigue siendo un importante problema de Salud Pública debido a las consecuencias que acarrea en los pacientes infectados. Entre los grupos de alto riesgo de adquirir este virus se encuentran los pacientes con enfermedad renal crónica (ERC) en tratamiento hemodialítico, por lo que la vacunación constituye un importante método de prevención y protección contra el virus de la hepatitis B (VHB). Objetivos: determinar el nivel de antiHBsAg y hallar la prevalencia de respuesta inadecuada a la vacuna contra la HB. Detectar los factores de riesgo asociados a la falta de respuesta inmunológica a la vacuna contra HB. Metodología: estudio observacional, descriptivo, prospectivo, de corte transverso, con componente analítico. Se incluyeron pacientes adultos con ERC en tratamiento hemodialítico trisemanal del Hospital Nacional de Itauguá durante el 2015. A todos se les determinó antiHBsAg. Resultados: se incluyeron 89 sujetos, de los cuales 47% tuvieron una respuesta inadecuada a la vacuna contra el VHB, con un leve predominio del sexo masculino. Conclusiones: la frecuencia de respuesta inadecuada a la vacuna es 47%. La edad, el hábito tabáquico, las comorbilidades y el estado de nutrición por IMC resultaron como factores de riesgo no significativos en la respuesta inadecuada a la vacuna contra el VHB, mientras que los años de hemodiálsis y la uremia se relacionaron de manera significativa a esta mala respuesta.Palabras claves: enfermedad renal crónica, antiHBsAg, vacuna hepatitis B, hemodiálisis AbstractIntroduction: Hepatitis B (HB) is still an Important problem of Public Health due to the consequences in the infected patients. Among the groups with high risk of acquiring this virus, there are the patients with chronic renal disease (CRD) receiving hemodialysis treatment. Vaccination is an important prevention and protection method against the hepatitis B virus (HBV). Objectives: To determine the level of anti-HBsAg, find the prevalence of inadequate response to the HB vaccine and detect the risk factors associated with the lack of immunological response to HB vaccine.
